4 Safety Consciousness. (Consider the Apprentice overall regard for Safety of himself and
Others, i.e. wearing of PPEs. General Safety work habits and cleanliness)
5 Cooperation. (Consider for Apprentice degree of cooperation towards his coworkers and other
who came in to contact with him in the course of performance of his duties.)
6 Reliability. (Consider how far can you depend upon the Apprentice for his reliability and
conscientiousness with or without supervision)
7 Adaptability. (Consider the Apprentice ability to adjust with the change conditions and emergent
situations.
10 Compliance with Rules & Procedures. (Consider Apprentices attitude for complying with
the companys policies, rules and procedures directly applicable to his conduct and job.)
