Strike Objective: To monitor, discover and apprehend suspected shoplifters.

 That on or about 0915H 02 Feb. 2018 while performing his duties and responsibilities
Mr. Jeffrey Biendima (Lane Personnel) inform me that he found out inside the case of
silver swan soy sauce 340ml one (1) bottle was lacking one to factory defect. The said
item was fully sealed and it was took picture for documentary purposes and resulting to
bad order (B.O).
 That on or about 1008H 03 Feb. 2018 One company named 3m Phils. (Scotch Brite)
conducted a picture taking of products inside the selling area and with permit form the
store supervisor.
 That on or about 1023H 03 Feb. 2018 one company named masa/PGP conducted a
picture taking of products inside the selling area and with permit from the store
supervisor.
 That on or about 1500H Feb. 2018 I issued an incident report to Andrai Mark Linggian
(Security Aide) because he was absent in the afternoon without permission.
 That on or about 0957H 04 Feb. 2018 I issued an incident report to Jovanie Sotelo (NPI
Diser) because he failed to monitor turn over to security aide his stock two (2) pieces
nescafe blend and bren orig 20g/12 with item code NP 0216worth of Php. 62.75 was
lacking and was expired last Nov. 30, 2017 found abandoned at excess shelves and two
(2) pieces nescafe 3in1 orig twin pack 34g/10s with item code NP7291 worth of Php.
75.45 was also lacking and also found abandoned at excess shelve before they pahse out
due to change packaging. The said items was charge to him.

 That on or about 1510H 14 Jan.2018 I issued an incident report to Marie Gel Molina and
Alvin Reyes (lane personnel’s) and Camilo Buis (Benby diser) because they failed to
display the stock of quaker oat diary drink vanilla malt flavor 250ml wherein it was
found 150 display at the shelve but have stock at the warehouse.
 That on or about 0810H 15 Jan.2018 I issued an incident report to Melvin Cacho (lane
personnel) because he failed to monitor and pull-out one (1) pack red bean cake with item
code pan 051 worth of Php. 50.50 was found expired Jan. 15,2018 displayed at lane #02.
 That on or about 1115H 18 jan. 2018 one company named Quanta Yaper Corp.
conducted a picture taking of products inside the selling area and with permit from the
store supervisor.
 That on about 1056H 20 Jan.2018 one company named 3m Phils. (Scotcal brite)
conducted a picture taking of products inside the selling area and with permit from the
store supervisor.
 That on about 1400H 20 Jan. 2018 I issued an incident report to Casper De La Paz (PGP
Diser) because he failed and he abandoned his stocksat lane #2.He way out and tends a
half day duty after his delivery.
 that on or about 0935H 21 Jan. 2018 one customer damaged one (1) piece Jolly spread
choco hazelnuts – 400g with item code fac 027 worth of Php. 112.30 we monitor it until
she gets to counter # 2 and pay the said damage. The cashier on duty was Annel Pauline
Valeros (cashier) transaction #356925.
 That on or about 1000H 21 Jan.2018 I issued an incident report to Harold Collado
(security aide) because of absent without permission (A.W.O.P)
