SUBJECT : Revised Form for Quarterly Report on All TB Cases

DATE : 16 September 2015

1. Reference: TB Implementation Program in Jails/Prisons.

2. Monitoring the status of the implementation of TB in Jails Program is one of the major
tasks that need to be accomplished by the Health JNCO or Jail Nurse/s. The number of
Persons Deprived of Liberty (PDL) with TB of all forms must be updated on a quarterly basis.

3. ITCON, here attached is the Revised Quarterly Report on all TB Cases for BJMP
Region VI ONLY. With this, the necessary information with regards to the subject above will be
comprehensive and updated. Reports must be submitted every 2nd day of the month after
each quarter. Below is the schedule of submission per quarter of the Quarterly Report on All
TB Cases:

4. Furthermore, the Quarterly Report on All TB Cases must be submitted through mail or e-
mail (bjmp6_hsu@yahoo.com) with the subject QUARTERLY REPORT ON ALL TB CASES.
The said report to be submitted though e-mail must be sent separately from other
reports/Memos (e.g. Monthly Health Report, Psychiatric communication/s, Request for Med
Supplies, etc.) to avoid misrepresentation of report. The jails with zero case of TB on a given
quarter are still expected to comply. An explanation letter is expected to the jails who failed to
submit the said report on a given deadline stated above.
