Individual’s overall ability to behave in a professional manner. Personal presentation: student wears appropriate attire with name identifier
and is well groomed. Greets client and confirms their identity. Obtains informed consent by introducing self (name and discipline),
explaining what treatment will be provided and why, and receives permission. Client is treated with respect and dignity (e.g. draping).
Performs assigned duties as instructed within capabilities.
I think I should score very good because I dressed and acted professionally through the whole session. I had
scrubs on, my hair pulled back, and my name badge on. I confirmed identity and received consent before
continuing, and throughout the session.
Performs environmental scan of treatment area and ensures personal and client safety. Uses standard precautions and general sanitization
techniques. All treatment materials are well planned, prepared and organized. Uses effective time management in preparing client, self and
environment. Client is well positioned for treatment. Uses good body mechanics. Good flow throughout the session.
I would rate myself good, I performed hand hygiene, I attempted to lower the table, but instead moved her to the
other table which was better at her level, and made sure her brakes her on. I think I could have connected the two
sections better to create better flow throughout the session.
Explanation and rationale is provided for treatment activities. Selection of treatment tasks are linked to client’s condition and goals.
Treatment components are carried out using safe, effective and efficient techniques. Client is the focus throughout treatment, not just the
task itself. Monitors client performance and response to treatment. Provides reinforcement, support and constructive feedback.
I gave myself good in this section, I connected what we were learning to her daily life and gave good rationale. I
monitored the client to ensure they understood the information by asking questions.

I rated myself good in this section, I thought about the table and moved her to a better one, I got her to put brakes
on. I recognised that they cushion was too inflated and that may be her cause of buttocks pain, and deflated to
proper depth, when I couldn’t get to the cushion from the back so I took the arm rest off in order to get a better feel.
In the communication section I gave myself a good, I tried by best to make the education more of a dialog, but
should have done more conversation. I should have slowed down and not talked so fast. I asked frequent
questions, but should have asked her specific questions about what we had learned to make sure she learned.

effectiveness. Meets minimum standards for progression. A few areas could be improved.
2 – Needs Improvement……………. Performance does not meet minimum standards. Errors or omissions compromised the
effectiveness of activity/exercise. Inconsistently performed, performance does not meet standards.
Significant improvement is needed before consideration for supervised patient care practice or
progression.
1 – Unsatisfactory/ Unsafe …......…. Performance indicates cause for concern. Performance is unsafe, ineffective and/or inappropriate.
Errors or omissions
compromise safety and/or significantly reduces treatment effectiveness. A potential for
compromised patient outcome or patient harm. Considerable improvement and remediation is
needed. Is not ready for supervised patient care practice or progression.

I think overall I did well, there were a few small changes I would make if I were to do it
again, but am happy with how I did.
