Contains Appointment details of the Customer with Eye Glass & Contact Lens Prescription
column to be filled in by the User. Prescription Details are displayed with space for
Optometrist to sign it.
Customer Delivery List
Available at the Store entails all the Trays that are Appointment Card
ready for Delivery to Customer. Displays Appointment Details like Customer Name,
Appointment Date & Time, Store, Store Location etc.
