Not wait for the first day of
school to submit proof of
immunizations.
➔ If your physician requests
immunizations to be given
after the first day of school,
a written note with the date
the vaccination will be given
is required.
➔ There is no specific form for
immunization records, your physician
will print out a copy of your child’s
most recent vaccinations at their
office.
➔ Please make sure before you leave the
doctor’s office that the immunization
record reflects what your child
received on the day of the visit.
➔ If your child has specific medical needs
or requires medication while at
school, I would be happy to meet with
you prior to the first day of school to
discuss and receive any necessary
medication.
