In accordance with the Code of Ethics of Arbitrators and as the nominated arbitrator in this
case, I truthfully made and execute the following disclosures. By executing this disclosure below I
do not ascribe, or intend to cause, any level of significance to the fact disclosed, or to recommend
that the disclosure might possibly affect my, or any arbitrator’s neutrality or impartiality. Instead,
the following disclosures are made for purposes of informing the parties and their duly
representatives of facts which in my judgment it is proper to disclose.

Alternative Dispute Resolution
6. I have no knowledge of any relationship of any kind with any of the parties to this
proceeding except to the CEO of the respondent corporation and I are members of the
professional organization and casually meet regularly during meetings and events of that
organization, also we belong to different sub-groups.
7. I disclosed in my personal profile that six months ago, I was previously served as Chairman
of the arbitral tribunal in CIAC arbitration. I rendered judgement on that case based on the
merits and evidence presented. The counsel for the claimant is also the counsel for the
herein respondent.
8. I am not aware and no idea as to who will be the witnesses for the CEO of the respondent.
Counsel and the parties in this proceeding are also encouraged to utilize some of the portals
to obtain further information about me, my law firm and firm’s practice. In any such event and
manner, that party or counsel desires that I provide more and further information regarding any of
the disclosures made above, I will do my earnest effort to do so.
I do not consider any of the foregoing disclosures as constituting either a conflict of interest
or raising an unjust and partial appearance to the judgement that I will later render. I am fully
confident that I can carry out my duties and obligations as a neutral and impartial arbitrator and
give each party to this case a just and reasonable hearing.
