AFFIDAVIT

I, ELIZABETH I ATILANO, of legal age, single, Filipino citizen and residing at B-337
Campo Islam, Zamboanga City, Philippines after having been sworn in accordance with law, do
hereby depose and state, That

That, I am a pensioner of the Armed Forces of the Philippines being the legal
wife of the late Sgt Bartolome D Balasulia PA who died on December 28, 1986;

That, I am informing the Commanding Officer, AFP Finance Center, GHQ AFP
and other officer concerned that I am now changing my residence address from
Tacloban City to my present address now at B-337 Campo Islam, Zamboanga City;

That, I am executing this affidavit attesting to the truth of the foregoing averment
and for whatever legal purpose it may serve.
