Purpose and method of this report

De La Salle Lipa’s Discipline Office held an re-orientation of students violation and offenses to
help the students become aware of the rules and regulation of the school proper. The purpose of
this report is to:
 Know the reasons why students keep on violating category 1 offenses.
 Analyze the students’ insight about the Discipline Office.
 Identify the most frequent offense the students violate.
 Decrease the number of violators in Category 1 offense.

We conducted a questionnaire survey to understand the reasons why students of De La Salle Lipa
still violates Category 1 offenses amidst the Discipline Office re-orientation every year. A total of
30 individuals responded to the survey.
