Perth Mint Excursion Consent Form

Dear Parent / Guardian,

The Year 5 class of CBD Primary School will be venturing on an excursion to the Perth Mint. This
excursion will link to the learning they have been doing in class relating to the Australian Gold Rush
and economics. The students will gain an insight into Western Australia’s history and the impact that
the discovery of gold has had on shaping Western Australian through the provided exhibits,
interpretive displays, informative guided heritage tour and demonstrations.

Date of the excursion: The excursion will take place on October 22nd, 2018.
Destination of the excursion: As a class we will be walking 15 minutes to and from the Perth Mint
(310 Hay Street, East Perth, WA, 6004)

Cost: $11.50 per student for the guided tour

Uniform: Students will be required to dress in their full sports uniform, including enclosed shoes and
hats.

What to bring:
A small bag containing:
• A small disposable recess that does not require refrigeration
• Hat
• Clipboard

If you are interested in helping on the day please advise your child’s classroom teacher.

CONSENT TO MEDICAL ATTENTION

Where the teacher in charge of the excursion is unable to contact me, I authorise the teacher in
charge to:
- Consent to my child receiving such medical or surgical attention as may be deemed necessary by a
medical professional
- Administer first aid as the teacher in charge may judge to be reasonably necessary.
