 Search for other example of physical hazards. Attacha photo that shows a situation of physical
hazards (state the source of photo).
 Describe the attached photo (e.g. types of physical hazards e.g. noise, radiation & vibration,
place, person involved, time)
 Identify the factors that cause the physical hazards.
 Suggest several strategies / countermeasures to prevent the physical hazards.
3. Save completed assessment sheet using the following format:
