PROJECT SUBMITTAL CHECKLIST


PURPOSE: The form DSA 3 submittal checklist is provided as a guide for submitting complete documents for a thorough
and comprehensive plan review. Complete submittals must include all applicable items listed on the form DSA 3, as
indicated in Procedure (PR) 17-03: Project Submittal Appointment Process. Non-conformance with these submittal
requirements will result in a failed submittal and rejection of the plan review application.
All items on the checklist that are applicable to the scope of the submitted project are required. Numerous items that were
previously required on earlier versions of the form DSA-3 are still required if applicable to the project scope, but will be
referenced as plan review comments and will be required prior to final approval and stamp out.
For projects that trigger Landscape Irrigation Water Efficiency Requirements, a form DSA 1-L: Outdoor Water Use Self-
Certification of Landscape Irrigation Design and Documentation, Site Landscape Area Location Plan, and fees are
required. Refer to DSA PR 15-03: Compliance with CALGreen Outdoor Water Use Regulations and items 9 and 10 on
the form DSA 3.
INSTRUCTIONS: All items indicated as “Required for All Projects at Time of Initial Submittal” must be provided for the
application to qualify as a complete submittal. Non-conformance with these requirements will result in a failed submittal
and rejection of the plan review application. The DSA 3 is a required submittal document and should include checks √
indicating required items included in the submittal.
Items listed under “Required Based on Plan Review Comments or Prior to Stamp Out” are required if merited by
the scope of the application. Applicants should make every effort to include these items at the time of original
application submittal, with the understanding that all or most of these items will be required prior to stamp out and
approval. Any items from this list not originally submitted will be required through the plan review comment process.
Some items not originally submitted may require submittal prior to back check appointment scheduling.

5. SITE PLAN AND/OR CIVIL DRAWINGS
Required for All Projects at Time of Initial Submittal:
A. Comprehensive campus site plan and enlarged site for scope of work
B. Identification of each building including name, use and occupancy
C. List the building type for buildings within the scope of work and adjacent buildings
D. If project is located in a flood hazard area, provide the required information on plans. Refer to 2016
California Building Code (CBC), Section 1603A.1.7 and PR 14-01 for required information.
E. If project is not located in a flood hazard area, provide a flood hazard map adopted by the local authority
supporting that position or supporting data per 2016 CBC, Section 1612A.3.
F. Within the scope of work, fully dimension real and assumed property lines from the face of buildings
G. Accessibility site plan data
H. Accessible parking spaces identified and detailed within scope
I. Parking ratio calculations for each parking lot, within or impacted by the scope of work
J. Accessible restrooms, drinking fountains, public phones and other required accessible features
K. Sidewalk and roadway delineated, with widths and surface materials identified within scope

required accessibility features

C. Provide room labels, indicating use and room occupancy with load factor used for occupant load
calculations identified (net or gross)
D. For each room or occupant area, indicate net or gross floor area totals

compartments, and fire-resistance-rated shafts, corridors, exterior walls and openings
H. For deferred or submitted bleachers, include a detailed bleacher seating layout, identifying accessible
seating and remaining floor area load calculations
I. Show fire-resistance-rated walls, with wall and detail schedules, including wall types, assemblies and
assembly design number references
J. Provide enlarged floor plans of restrooms, elevators, stairs, ramps, lifts, etc., cross-referenced
and dimensioned
K. Elevator location in relation to nearest stair identified within 200 feet
L. Indicate fixed furniture and equipment layouts, detailing casework functions and accessibility features

F. Provide door, hardware, windows and wall finish schedules, including details, panic hardware, fire doors,
referencing any fire-resistance-rated and tempered glazing/window assemblies
G. Signage schedules, including details of room identification and way-finding signage
H. Casework details, including elevations, details, anchorage and required accessibility clearances
and features
I. Identify soffits and other architectural projections
J. Provide all mounted equipment brackets and detail references
K. Identify walk-in refrigerators and freezers with detail references
L. Identify roof classes for new and existing roofs

Required for All Projects at Time of Initial Submittal:
A. For projects exempt from the Green Oaks Family Academy Elementary School Fire Protection Act (SB
575, Chapter 725, Statutes of 2001), provide a letter signed by the school district superintendent stating
the project is exempt, and place a similar statement on the fire alarm title sheet
B. Provide an automatic fire alarm system for all new buildings at a new or existing campus and for
modernizations if project cost exceeds $200,000 with any state funding

H. Provide State Fire Marshal listings, specifications and manufacturer’s product data sheets for all AFSS
materials and devices
PART 4 – CALCULATIONS AND REPORTS:
15. PROJECT STRUCTURAL CALCULATIONS
Required for All Projects at Time of Initial Submittal:
A. Provide one set of stamped and signed structural calculations
B. Include an index of all calculations
C. Provide a scope of work covered by the submitted calculations
D. Indicate the seismic, wind and importance load factors used in the calculations
E. Indicate the complete design criteria
F. Indicate the snow load utilized in the design
G. Provide snow drift calculations, if appropriate
H. Indicate utilized soil bearing pressure. If greater than 1,500 psf, provide substantiating
geotechnical report.
I. Indicate utilized lateral soil bearing pressure. If greater than 100 psf, provide substantiating
geotechnical report.
J. Provide allowable lateral soil pressure for the design of poles, signs or antennae
K. Provide structural plans for foundations, floors and roofs, coordinated and cross referenced to the
submitted structural calculations
L. Provide lateral drift calculations as required by code
M. Provide load calculations, including weight of mechanical and electrical units and fire sprinkler pipe
N. Provide calculations for mechanical equipment anchorage, including overturning
O. Provide complete truss calculations and details for open-web trusses, unless deferred

T. Identify rigid diaphragms and provide calculations for accidental torsion consideration
U. Provide dynamic analysis calculations for buildings with structural irregularities, in accordance with
ASCE Table 12.6-1
V. For designs by computer analysis, include a representative hard copy with input and output files on a CD
W. Provide calculations for site structures: light poles, signs, antennas, retaining walls, etc.

X. Provide calculations demonstrating that the building mass has not increased by more than 10 percent,
and the lateral resisting capacity has not been reduced by more than 5 percent
Y. Provide justification that the cost of the building alterations does not exceed 50 percent of the building
replacement cost
16. GEOTECHNICAL INVESTIGATION AND GEO-HAZARDS REPORT
Required for All Projects at Time of Initial Submittal:
A. Provide two copies of a Geotechnical Investigation “Soils Report” with the appropriate professionals’
stamps and signatures
B. Provide one copy of the Geo-Hazards Report, stamped and signed, submitted with a copy of a
transmittal to the California Geological Survey (CGS), per IR A-4
C. If reports are older than two years, provide a letter updating original report(s) by the same geotechnical
engineer or geotechnical engineering firm
D. All reports to be applicable to the buildings in the submitted scope of work
E. Geo-Hazards statement signature on the form DSA 1(for existing school sites only)
F. Copy of site data report submitted to CGS per CBC 1603A.2
