SUBJECT: Notice of Termination of Employment

Dear Ms. XXX,

This has reference to your failure to monitor Collection Reps that willfully processed numerous
payments for customers without calling them or talking to them while doing the transactions from
January 2010 until April 2010. These purported nefarious activities and fraudulent scheme went
undetected by you throughout that time.

In response to the show-cause letter we sent you on date to explain why no disciplinary sanctions
should be imposed upon you, you admitted in your letter dated May 31, 2010 and during the
Admin Hearing conducted on June 3, 2010 to not being aware that such activities existed from
the time you started handling the Collections Team.

Such failure committed by you constitutes gross negligence, which according to Article 282 of the
Labor Code is a valid ground for termination.

In view of the foregoing, we regret that we are terminating your services effective on June 15,
2010.
