AFFIDAVIT OF DISCREPANCY

I, REX PATARATA SORIANO, of legal age, Filipino, with address c/o Fortun
and Santos Law Office 134 CRM Avenue BF Almanza, Las Pinas, Philippines, after having
been duly sworn to in accordance with law, do hereby depose and state the following to
wit:

1. I was born on September 04,1972 in Brgy Dulao, Bago City, Negros Occidental to
Spouses Hernane Varona Patarata and Rebecca Bayona Soriano.

2. Ever since of my existence, I knew that my birthdate was September 04, 1972.

3. I was able to secure a copy of my birth certificate from the Local Civil Registrar of
Bago, Negros Occidental and said document reveals that I was born on September
04, 1972.

(A copy of my Certificate of Live Birth issued by Local Civil Registrar is herein

attached as Annex ‘A’, for easy reference)

4. Surprisingly, when I requested for the first time a copy of my Certificate of Live
Birth from the Philippine Statistics Authority to be submitted as a requirement for
application of a driver’s license, I discovered that my birthdate was July 30, 1972.

(A copy of my Certificate of Live Birth issued by Philippine Statistics Office is herein

attached as Annex ‘B’)

5. All my identification cards reflects that my birthdate was September 4, 1972 and
not July 30, 1972.

(A copy of my Philheath card, HDMF, Private Security License Card and Unified
Multipurpose ID are herein attached as Annex C and series, for easy reference)

6. That the discrepancy abovementioned is a clerical error;

7. That I further declare that from now on, I would like to correct my birth date in
my Certificate of Live Birth as September 4, 1972 to appear in all my documents
to avoid confusion.

8. That I am executing this instrument to attest to the truth of the foregoing

statements and for all legal intents and purposes it may serve.
