Details of incident account (Translated into English):

- Hari isnin, 8/10/18, pukul 8.00 lebih saya masuk ke control room and plant dalam process
start-up, masa itu turbine sudah full speed no load, 3000 rpm.
At around 0800 hours on Monday, 8 October 2018, I went into the control room and the
plant was in the process of starting up. The turbine was at full speed no load, 3,000 rpm.

- Ada beberapa orang yang monitor process start-up.

There were a few people monitoring the start-up process.

- Saya duduk dalam control room dan dalam process issue permit untuk hot commissioning
dalam pukul 9.00 lebih.
I sat in the control room and was in the process to issue permit for hot commissioning at
around 0900 hours.

- Masa buat hot commissioning, pukul 11.00 pagi lebih kurang, turbine tripped on generator
protection, dan rpm jatuh dari 3000 sehingga rpm kurang.
During the hot commissioning, at around 1100 hours, the turbine tripped on generator
protection and rpm dropped from 3,000 rpm until the rpm was insufficient.

- Saya pun pergi ke AVR room dan tanya En Manesh dan En Jeff dari GE tentang apa yang
berlaku dan mereka kata mereka activate generator protection itu dan menyebabkan
turbine itu trip.

I went to the AVR room and asked Mr Manesh and Mr Jeff from GE about the incident and
they informed that they have activated the generator protection which caused the turbine
to tripped.

- Sebelum itu, mereka deactivated generator protection itu sebab itu sebahagian dari
procedure.
Before that, they deactivated the generator protection as it was a part of the procedure.

- Agak pukul 11.30 pagi, mereka panggil untuk restart balik turbine.
At around 1130 hours, they asked to restart the turbine.

- Lepas itu, saya perlu balik ke CCR dan mereka sambung balik hot commissioning dan
turbine ada sampai 3000 rpm.
After that, I went back to CCR and they resume the hot commissioning process and the
turbine reach 3,000 rpm.

- Lepas kurang pukul 2.00 petang, saya ada dalam surau, satu bilik dalam CCR.
At around 1400 hours, I was in the praying room which is located within the CCR as well.

- Masa sembahyang, saya dengar bunyi siren di seluruh plant.

As I was praying, I heard the siren at the entire plant.

- Lepas itu, saya pergi ke CCR dan SUS, En Anuarizol beritahu ada berlaku kebakaran dalam
AVR room.
I went to CCR and SUS after that and Mr Anuarizol informed that there was a fire in the AVR
room.

- Saya nampak ada detector sudah activate dalam AVR room di fire alarm control panel.
At the fire alarm control panel, I saw the detector in the AVR room was activated.

- Lepas itu, Head of Shift, En Hairol Kamarudin instruct untuk activate emergency stop
button untuk berhentikan turbine.

After that, the Head of Shift, Mr Hairol Kamarudin instructed to activate the emergency
stop button to stop the turbine.

- Kemudian, saya call bomba dari station Pontian untuk emergency support.
I called the Pontian Fire and Rescue Department for emergency support after that.

- Hari Isnin, saya masuk kerja 0800 pagi sebab nak buat testing full speed (full current) no
load, turbine sampai 3000 rpm oleh GE, thyristor by thyristor.
On Monday, I clocked in at 0800 hours because wanted to do full speed (full current) testing
with no load, turbine reach 3,000 rpm by GE, thyristor by thyristor.

- Baru hari pertama nak buat testing pada AVR.

It was the first day to do testing on AVR.

- Lepas itu, kami buat IR test (meggar test) pada cable (3 cables) untuk excitation transformer
ke AVR dengan GE.
After that, we did IR test (meggar test) on three (3) cables for the excitation transformer to
AVR with GE.

- Hasil testing tidak ada masalah dengan cables.

The result of the testing was that there were no problems with the cables.

- Lebih kurang pukul 1400 petang, GE sedang buat testing thyristor (TB), bridge 1 dan sudah
nak habis.
At around 1400 ours, GE came to do testing of the thyristor (TB), bridge 1 and almost
completed.

- GE sedang testing Stage 4, current test at full speed no load untuk TB2.
GE was testing Stage 4, current test at full speed with no load for TB2.

- Masa itu, saya ada di bawah platform floor 8m.

That time, I was below platform floor 8m.

- Tiba-tiba, saya terdengar bunyi letupan 2 kali dari AVR room dan nampak asap kelabu
keluar dari AVR room.
Suddenly, I heard two (2) explosion sound from the AVR room and saw grey smoke
emanating from the AVR room.

- Masa itu, alarm sudah bunyi sebab ada smoke detector dalam AVR room.
The alarm was activated as there was smoke detector in the AVR room.

- Memang tidak nampak ada api.

Did not saw fire.

- Pintu AVR room setengah terbuka sebelum letupan, lepas letupan, En Manesh dari GE
tutup pintu itu.
The door of the AVR room was partially open before the explosion, Mr Manesh from GE
closed the door after the explosion.

- Saya segera hubungi ERT dan ERT sampai beberapa minit kemudian.
I immediately called ERT and ERT arrived several minutes later.

- Bila ERT sampai, mereka buka pintu dan hanya nampak asap saja, tidak ada apa-apa api
semasa itu.
When ERT arrive, they open the door and saw a lot of smoke only, there was no fire that
time.

- Bomba dari Pontian Station sampai 40 minit dan nampak yang situasi memang sudah
terkawal.
The Pontian Fire and Rescue Department arrived forty (40) minutes later and saw the
situation was already under control.
