1.0 Purpose

This Construction Division Standard is established to outline the safe operating procedures
for several activities with higher risks and general occupational health and safety
requirements at any workplace or construction site. The activities and general occupational
health and safety requirements identified are as follows:

2.0 Responsibilities

2.1 The Person in Charge is responsible to ensure the requirements of this procedure on related
activity and general safety and health requirements are effectively implemented at the
workplace.

2.2 The project team shall be responsible to establish and document the specific procedure for
construction activities/ requirements that are not specified in the list above.

3.0 Procedures

3.1 Blasting Work

3.1.1 All blasting works shall obtain Permit To Work from IJM authorized
personnel and government departments such as Department of Minerals and
Geosciences, Police and other concerning Authority before commencing
work.

3.1.2 The permit to work for blasting works is valid per blasting work

3.1.3 Explosives shall not be handled or used except by the competent person or
Blasting Consultant. The competent person / Consultant shall: -

a. Give advice on the security and safety aspects of the blasting works.
b. Prepare the method statement.
c. Responsible on overall blasting works.

3.1.4 To appoint a Blaster / Shot Firer who shall then:

a. Obtain the permit or approval from Department of Minerals and


Geosciences and Police.
b. Obtain the license or approval (from Police and/or other Authority) for
buying, carrying and using the explosive material

3.1.5 The neighboring projects/premises should be notified on the blasting works


and get their feedback. All minutes of meeting should be compiled.

3.1.6 The layout plan of the blasting area including the danger zone, evacuation
point and distance of the nearest structure should be established.

3.1.7 The blasting checklist should be signed (before blasting work carried out) by:
-

a. The Blaster/Shot Firer.


b. Blasting Consultant / competent person.
c. Safety and Health Officer.
d. Project Management Team.

3.1.8 When explosives are delivered to site, the blasting area shall not be accessible
to other workers except the blasting crew. The blasting area shall be cordoned
off with warning tapes.

3.1.9 No radio or mobile phones are allowed to be operated within 10m from the
blasting area.

3.1.10 Report shall be prepared and compiled after each blasting.

3.1.11 At the end of the blasting operation any excess explosive shall be returned to
the explosive supplier or magazine under police escort.

3.1.12 The blasting work should conform to the Explosive Act.

3.1.13 Permit To Work shall be displayed at the workplace.

3.2 Confined Space Entry

3.2.1 Confined space means, a space which: -

a. Is not intended as regular workplace (i.e. continuous employee


occupancy).
b. Has restricted means of entry or exit.
c. Is large enough and so confined that an employee can bodily enter and
perform assigned work.
d. Is at atmospheric pressure during occupancy.

3.2.2 In addition, this space has at least one of these characteristics;

a. It contains or has a potential to contain a hazardous atmosphere.


b. It contains material (solid or fluid) that has potential for engulfing an
entrant.
c. It has an internal configuration such that an entrant could be trapped or
asphyxiated by inwardly converging wall or by a floor which slopes
down-ward and tapers to a smaller cross-section.
d. It contains any other recognized serious safety or health hazards, for
example rotors.

3.2.3 All works in a ‘Confined Space’ shall obtain ‘Permit To Work’ from IJM
authorized person / other authority before commencing work.

3.2.4 The permit to work for working in a confined space is valid for maximum
twelve (12) hours only.

3.2.5 No person / worker is allowed / permitted to enter the confined space unless:-

a. All practicable steps have been taken to remove any fumes which may be
present and to prevent any ingress of fumes.
b. It has been ascertained by a suitable test that the confined space is free
from dangerous fumes.
c. A reliable and competent person is stationed / stand-by at the entrance to
the confined space to supervise the operation and take such action as may
be necessary in any emergency.

3.2.6 If such test cannot be carried out, the person / worker entering the confined
space shall wear efficient respiratory protective equipment suitable for the
dangerous substance that is to be expected and a suitable safety harness with

attached lifeline of adequate dimensions leading to a convenient point outside


the confined space.

3.2.7 Special suit (if necessary) shall be worn suitable for protection against the
presence of toxic / harmful chemical.

3.2.8 Employees to be involved in the confined space operations shall be trained in


confined space safety procedure.

3.2.9 A suitable and proper warning sign / notice indicating the nature of danger
shall be posted prominently nearby.

3.2.10 Any hot work carried out in confined space shall obtain hot PTW before
commencing work and the Operator / Welder shall be a competent person.

3.2.11 Regulation 13 of the ‘Factory And Machinery (Safety, Health And Welfare)
Regulation 1970 and ‘Code of Practice For Safe Working In A Confined
Space’ by DOSH are applicable.

3.2.12 Permit To Work shall be displayed at workplace.

3.3 Crane Operation

3.3.1 All crane operation include mechanical lifting shall obtain Permit To Work
from IJM authorized person before commencing work.

3.3.2 The permit to work for crane operation is valid for maximum 7 working days
only.

3.3.3 No crane is allowed to be used at the workplace without proper and valid
certificate of fitness (PMA).

3.3.4 All operators shall be competent and registered with Department of


Occupational Safety and Health (DOSH).

3.3.5 All cranes (and its equipments) shall be inspected as follows:

a. Pre-entrance inspection by Safety Personnel, Operator, Site Supervisor.


b. Daily inspection by Operator or Rigger or Signalman.
c. Monthly inspection by competent / designated person to apply colour
coding for slings etc.
d. Log book should be maintained.

3.3.6 Any lifting operation (with load more than 0.5 tons) shall be under supervision
of designated person.

3.3.7 Signalman shall be assigned to assist the crane operator and control the
movement of load.

3.3.8 Use suitable sling to the particular load.

3.3.9 Test certificate issued by manufacturer shall be made available for all slings.

3.3.10 Safe Working Load of lifting tools and slings shall be checked before any
lifting.

3.3.11 To provide timber packing for the unstable ground (for mobile/crawler crane),
outrigger must be fully extended, padded and tested.

3.3.12 All timber packing should be sound, free from defects, of ample and adequate
strength.

3.3.13 Tip load, lifting angel and radius (as per load chart) shall be strictly complied
with.

3.3.14 Permit To Work shall be displayed at the workplace.

3.4 Demolition Work

3.4.1 All demolition work (existing building or building under construction


involving the structure or the whole wall or whole floor or any mechanical
method of demolition) shall obtain Permit To Work from IJM authorized
personnel and other authority such as local authority before commencing
work.

3.4.2 The permit to work for demolition works is valid for maximum 7 working
days only.

3.4.3 Any demolition work shall: -

a. Provide the proper warning/danger sign around the property.


b. Provide the proper barricade around the building/premises or property.
c. All doorways or thoroughfares giving access to the property shall be kept
barricaded except when being used as a passage for men or equipment and
during darkness and lights at flares shall be placed on or around all
barricades.

3.4.4 Before commencing any demolition work:-

a. All glass in exterior operation shall be removed.


b. All gas, electric, water, steam and other supply lines shall be shut off and
capped.
c. The relevant authorities involved shall be notified in advance.
d. Where it is necessary to maintain any power, water, gas or electric lines
during demolition such lines shall be so re-located or protected with

substantial covering so as to protect them from damage and to afford to


the employees.

3.4.5 During the demolition of any structure the work contractor shall examine the
walls of all structures adjacent to the structure, which is to be demolished.

3.4.6 Where there are reasons to believe that an adjacent structure is unsafe or will
become unsafe because of demolition operations, no demolition shall be
performed at this point until there has been provided sheet piling, barricade or
other such means as may be necessary to ensure the stability of the adjacent
structure and to prevent such structure or other property from collapsing.

3.4.7 Demolition of walls and partitions, it shall: -

a. Proceed in a systematic manner and all works above each tier of floor
beams shall be completed before the safety of its supports is impaired.
b. Masonry shall neither be loosened nor permitted to fall in such masses as
to endanger the structural stability of any floor or structure support.
c. No wall, chimney or other structure or part of a structure shall be left
unguarded in such a condition that it may fall, collapse or weaken due to
wind pressure or vibration.
d. In the demolition by hand of exterior walls, safe footing / safe working
platform for the workers shall be provided in the form of sound flooring
or scaffolds.
e. Walls or partitions, which are to be demolished by hand, shall not be left
standing more than one story high above the uppermost floor on which
men are working.

3.4.8 During demolition, continuing inspections shall be made by the authorized /


designated person as the work progresses to detect any hazard to the workers
resulting from weakened or deteriorated floors or walls, or loosened material.
No worker shall be suffered or permitted to work where such hazards exist
unless they are corrected by shoring, bracing or other effective means.

3.4.9 The use of a swinging weight, clamshell, bucket, power shovel, bulldozer, or
other contrivances for the purpose of demolition works shall be in accordance
with the following requirements:-

a. The building or structure or remaining portion thereof shall be not more


than 24 meters in height.
b. Where the swinging weight is used, a zone of demolition having a radius
of at least one and a half times the height of the structure or portion
thereof being demolished shall be maintained around the points of impact.
c. Where the clamshell bucket is being used, a zone of demolition shall be
maintained within 8 meters of the line of travel of the bucket.
d. Where the other contrivances are being used to effect total or partial
collapse, there shall be maintained in the area into which the affected

portion may fall a zone of demolition at least one and half times the height
of the structures or remaining portion.
e. No person other than workers essential to the operation of the equipment
shall be permitted to enter a zone of demolition, which shall be provided
with substantial barricade.

3.4.10 Part XI of Factories And Machinery (Building Operations And Works Of


Engineering Construction) (Safety) Regulations 1986 and MS 2318: 2010 (P)
– Demolition of Buildings-Code of Practice (First Revision) are applicable.

3.4.11 Permit To Work shall be displayed at the workplace.

3.5 Hot Work

3.5.1 All hot work activities (welding and cutting) carried out inside closed area or
nearby inflammable / combustible material and burning work shall obtain
‘Permit To Work’ from IJM authorized person / other authorities before
commencing work.

3.5.2 The permit to work for hot work is valid for maximum 30 working days only.

3.5.3 The Operator / Welder shall be competent in the field of work.

3.5.4 All hot work equipment shall be in good condition and checked and satisfied
by the safety personnel.

3.5.5 Cylinder regulator shall be provided / equipped with ‘Flash back’ arrestor.

3.5.6 Cylinders shall be secured in up-right manner and stored in a suitable cylinder
rack / trolley.

3.5.7 Cylinder shall be kept away (at least 3 meter) from the actual hot work
operation area and shall not be located in close area.

3.5.8 Empty / unwanted cylinder shall be taken away from the hot work operation
area and stored at proper designated storage area.

3.5.9 The fuel and oxygen hoses shall not be laid over sharp edges or hot material.

3.5.10 To provide (if necessary) the fire resistance material (Fire blanket) to shield
the inflammable/ combustible material or to protect the worker from the
welding spark.

3.5.11 All workers shall be provided with proper personnel protective equipment
such as eyes protection / face shield, hand glove, safety helmet, proper
footwear etc.

3.5.12 Adequate number of fire extinguisher (at least ABC type) must be available at
the hot work operation area (Within 3 to 6 meters).

3.5.13 All gas cylinders / hot work equipment shall be properly labeled to indicate
the Work Contractor / site operative company’s name.

3.5.14 Any hot work in the confined space is prohibited unless permitted by the
authorized person.

3.5.15 Permit To Work shall be displayed at the workplace.

3.6 Handling of Nail Gun

3.6.1 All works using nail gun/ramset fastener shall obtain Permit To Work from
IJM authorized personnel and other Authorities such as Police before
commencing work.

3.6.2 Nail gun / Ramset fastener shall only be used by the competent and licensed
person. The permit is valid for the entire work operation (unless the person is
replaced or the competency certificate becomes invalid).

3.6.3 Proper safety warning sign shall be conspicuously posted around the work
area.

3.6.4 The work area shall be barricaded.

3.6.5 Only authorized workers are allowed to enter the work area.

3.6.6 Special personal protective equipment such as high impact ear protection and
high impact resistant eye protection shall be provided to the workers.

3.6.7 Project shall comply with all safety procedures pertaining to the explosive
material.

3.6.8 Permit To Work shall be displayed at the workplace.

3.7 Working under or near Power Lines

3.7.1 All works under/near the power lines shall obtain Permit To Work from IJM
and Tenaga Nasional Berhad (TNB) authorized personnel before commencing
work.

3.7.2 The permit to work for working under or near power line validity shall follow
the TNB approval.

3.7.3 All works/activities carried out under the power lines shall be under
supervision by a competent Engineer.

3.7.4 Proper personnel protective equipment shall be provided.

3.7.5 Permit To Work shall be displayed or made available at the workplace.

3.8 Radiation Exposure

3.8.1 Any work/activity involving ‘Radiation Exposure’ shall obtain Permit To


Work from IJM authorized personnel before commencing works. The permit
to work for radiation work is valid for maximum 7 working days only.

3.8.2 Only competent and licensed person is allowed to handle any equipment
containing or emitting radiation hazardous to health.

3.8.3 The work area shall be barricaded and isolated in order to establish the
controlled area.

3.8.4 Only authorized person is allowed to enter the work area.

3.8.5 Proper safety warning sign shall be conspicuously posted around the area.

3.8.6 Proper and adequate personal protective equipment e.g. respiratory protection
shall be provided.

3.8.7 Workers who work in the ‘controlled’ area shall be closely supervised.

3.8.8 Proper hygiene measures such as washing and showers shall be provided.

3.8.9 The basic protection measure associated with time, distance, and shielding
shall be complied and implemented.

3.8.10 Permit To Work shall be displayed at the workplace.

3.9 Working at Height

3.9.1 All working at height works (more than 3.0 meters above ground / floor or any
external works) must obtain ‘Permit To Work’ from IJM authorized person
before commencing work. The permit to work for working at height is valid
for maximum 7 working days only.

3.9.2 All working at height works shall be provided with proper and safe working
platform. Such working platform (either permanent or temporary) shall be
provided with :-

a. Proper safety railing or guardrail of adequate strength to a height of at


least one meter above the platform or place and above any raised standing
place on the platform.

b. Proper toe-boards up to a sufficient height being in no case less than 200


mm and so placed as to prevent so far as possible the fall of person,
materials and tool from such platform or place.

3.9.3 It also shall be provided with proper and safe access and egress to and from
work area. Such access and egress shall be provided with:-

a. Proper landing / half-landing (for more than 9 meter).


b. Proper handrail and handhold (the ladder shall be raised to a height of at
least one meter above the place of landing).
c. Adequate overhead protective cover in order to protect worker entering or
leaving the working area.
d. The ladder shall be secured to prevent swaying.
e. The slope of the ladder should base on ratio 4:1 (Vertical: Horizontal).

3.9.4 A proper and safe ‘Life Line’ should be provided if proper working platform
cannot be erected.

3.9.5 Where work is being performed on roofs having a slope greater than 30°, the
protection against sliding consisting of roofing bracket or crawling board
should be provided.

3.9.6 The roofing bracket shall be:-

a. Constructed to fit the pitch of the roof and when in use shall provide a
level-working platform.
b. Secured in place by nailing pointed metal projections attached to the
underside of the bracket and securely driven into the roof or by a secure
rope passed over the ridgepole and tied.
c. The crawling board shall not be less than 250 mm wide and 25 mm thick
and shall have cleats at least 38mm wide, spaced at equal intervals not
more than 310 mm apart across the full width of the board and firmly
nailed. Such boards shall extend from the ridgepole to the eaves when
used in connection with roof construction, repair or maintenance.

3.9.7 Safety harness shall be provided to workers conducting work at height.

3.9.8 Permit To Work shall be displayed at the workplace.

3.10 Excavation Work

3.10.1 All excavation works shall obtain ‘Permit To Work’ from IJM authorized
personnel / other authority before commencing work. The permit to work for
excavation work is valid till completion of the activity unless there are any
changes of condition.

3.10.2 Any excavation at/near the ‘Public Area’ shall be provided with:-

a. Public walkways, sidewalks with substantial guardrails or board fences.


b. A flagman or watchman to warn the public on the movement of
vehicle/trucks in/out of the property.
c. All public sidewalks shall be adequately illuminated and warning light or
flares shall be placed around the property to ensure safety of pedestrian
and vehicular traffic.

3.10.3 Professional Engineer shall design the support of the adjoining structure as
underpinning, sheet piling, shoring or bracing.

3.10.4 The planks used as sheet piling shall be at least 50 mm thick. The maximum
spacing between wales shall be within their safe bending stress. Shores and
braces shall be of adequate dimensions for stiffness and shall be placed as to
be effective for their intended purposes. Each end of each wales piece shall be
separately braced.

3.10.5 Earth-supported shores or braces shall bear against a footing of sufficient area
and stability to prevent their shifting.

3.10.6 Excavation wall shall be cut in such manner to provide safe and stable slope
or benching against risk of failure and collapse. At area where safe and stable
slope or benching is not practical due to limited area or unstable soil, the
excavation work shall be protected with provision of sheet piling, shoring or
other measures and no worker shall be permitted to enter such excavation area
in the absence of such protection measure.

3.10.7 A designated person shall check the excavation site and its vicinity after every
rainstorm or other hazard-increasing occurrence and the protection against
slide and cave-ins shall be increased, if necessary.

3.10.8 The temporary sheet piling installed to permit the construction of a retaining
wall shall not be removed until the wall has developed its full strength.

3.10.9 Where the banks are undercut adequate shoring shall be provided to support
the over changing materials.

3.10.10 Checking shall be carried out to detect underground utilities e.g. power
electrical cable (TNB), fibre optic cables, water pipelines, gas pipelines,
telecommunication cables, sewerage pipe etc. before any excavation is
conducted.

3.10.11 Excavated materials and other superimposed loads shall be placed at least
610 millimeters from the edge of open excavation and trenches, and shall be
so piled or retained that no part thereof can fall into the excavation, or cause
the banks to slip or cause the upheaval of the excavation bed.

3.10.12 Banks shall be stripped of loose rocks or other materials, which may slide,
roll or fall upon persons below.

3.10.13 Adequate barricades shall guard open sides of excavations where a person
may fall more than 3 meters and suitable warning signs shall be put up at
conspicuous position.

3.10.14 No worker shall be permitted to work where he may be struck or endangered


by an excavating machine or by material dislodged by it or falling from it.

3.10.15 Sufficient number of proper and safe access and egress such as ladder,
stairways or ramp shall be provided in every excavation of more than 1.2
meter deep.

3.10.16 Permit To Work shall be displayed at the workplace.

3.11 Fire Prevention

3.11.1 All appropriate measures shall be taken by the employer to:


(a) avoid the risk of fire;
(b) control quickly and efficiently any outbreak of fire;
(c) bring about a quick and safe evacuation of persons.

3.11.2 Sufficient and suitable storage shall be provided for flammable liquids,
solids and gases. Gasoline, volatile solvents or any other flammable
substances must be stored in containers that are clearly labeled, approved for
their contents and located in a safe place away from any source of open flame.

3.11.3 Secure storage areas shall be provided for flammable liquids, solids and
gases such as liquefied petroleum gas cylinders, paints and other such
materials in order to deter trespassers.

3.11.4 Smoking shall be prohibited and "No Smoking" notices be prominently


displayed in all places containing readily combustible or flammable materials.

3.11.5 Burning of rubbish or builders waste is not permitted on site. Open fires of
any type are not permitted.

3.11.6 In confined spaces and other places in which flammable gases, vapours or
dusts can cause danger:
(a) only suitable protected electrical installations and equipment,
including portable lamps, shall be used;
(b) there shall be no naked flames or similar means of ignition;
(c) there shall be notices prohibiting smoking;
(d) oily rags, waste and clothes or other substances liable to
spontaneous ignition shall be removed without delay to a safe place;
(e) adequate ventilation shall be provided.

3.11.7 Combustible materials such as packing materials, sawdust, greasy/oily


waste and scrap wood or plastics shall not be allowed to accumulate in
workplaces but shall be kept in closed metal containers in a safe place.

3.11.8 Regular inspections shall be conducted for places where there are fire
risks. This includes the vicinity of heating appliances, electrical installations
and conductors, stores of flammable and combustible materials, hot welding
and cutting operations.

3.11.9 Welding, flame cutting and other hot works shall only be done on the
orders of a competent supervisor after appropriate precautions, as required,
are taken to reduce the risk of fire.

3.11.10 Places where workers are employed shall, if necessary to prevent the
danger of fire, be provided as far as practicable with:
(a) Suitable and sufficient fire-extinguishing equipment, which shall be
easily visible and accessible;
(b) An adequate water supply at ample pressure.

3.11.11 Fire-extinguishing equipment shall be properly maintained and inspected at


suitable intervals by a competent person. Access to fire-extinguishing
equipment such as riser, portable extinguishers and connections for hoses
shall be kept clear at all times.

3.11.12 All supervisors and a sufficient number of workers shall be trained in the use
of fire-extinguishing equipment, so that adequate trained personnel are readily
available during all working periods.

3.11.13 Where necessary to guard against danger, workers shall be suitably trained in
the action to be taken in the event of fire, including the use of means of
escape.

3.11.14 Where appropriate, suitable visual signs shall be provided to c l e a r l y


indicate the direction of escape in case of fire.

3.11.15 Means of escape shall be kept clear at all times. Escape routes shall be
frequently inspected particularly in high structures and where access is
restricted, as in tunnel workings.

3.11.16 Sufficient and suitable means to give warning in case of fire shall be
provided where this is necessary to prevent danger. Such warning shall be
clearly audible in all parts of the site where persons are liable to work.
There shall be an effective evacuation plan so that all persons are
evacuated swiftly without panic and accounted for and all plant and
processes shut down.

3.11.17 Notices shall be posted at conspicuous places indicating:

(a) the nearest fire alarm;


(b) the telephone number and address of the nearest emergency services.

3.12 Means of Access and Egress

3.12.1 Access and egress to all exits, fire and safety equipment, and work areas must
be kept clear of obstructions at all times. Special attention must be given in
maintaining clear walkways, removal of trash, removal of slipping and
tripping hazards, and proper storage of materials.

3.12.2 All buildings under construction of more than 2 storeys high shall be provided
with well-defined access at the ground floor with adequate overhead
protective cover.

3.12.3 Overhead protective cover shall be constructed using materials that have the
required strength to sustain the impact of object falling from height.

3.12.4 The overhead protection cover shall extend to a minimum distance of 3 meter
from the building.

3.12.5 All staircases being used for access shall be provided with adequate railings at
all exposed sides.

3.12.6 Workers must be instructed not to stack/store material at access, staircases and
walkways.

3.13 Lighting/Illumination

3.13.1 Where natural lighting is not adequate to ensure safe working conditions,
adequate and suitable lighting, including portable lighting where appropriate,
shall be provided at every workplace and any other place on the construction
site where a worker may have to pass.

3.13.2 Artificial lighting shall, as far as practicable, not produce glare or


disturbing shadows.

3.13.3 Where necessary to prevent danger, lamps shall be protected by suitable


guards against accidental breakage.

3.13.4 The cables of portable electrical lighting equipment shall be of adequate size
and characteristics for the power requirements and of adequate mechanical
strength to withstand severe conditions in construction operations.

3.13.5 For passageway, stairways and landings, the illumination shall not be less than
50 lux.

3.13.6 Provisions shall be inspected frequently to ensure in working condition at all


times.

3.14 Rubbish Removal/Housekeeping

3.14.1 All rubbish must be removed daily from work area to the designated points
before end of work.

3.14.2 All trade/sub-contractor shall submit housekeeping program to IJM prior to


commencement of activity.

3.14.3 Program shall outline the responsibilities, manpower, tools, equipments and
time frame.

3.14.4 The housekeeping program shall be implemented and monitored on its


effectiveness.

3.14.5 Acceptance of sub-contractor’s work completion shall include performance on


housekeeping.

3.14.6 Person In Charge of workplace shall organize group housekeeping (Gotong-


Royong) if the condition of the workplace is not satisfactory.

3.15 Electricity

3.15.1 All electrical installations shall comply with Federal Government and SIRIM
standards, current regulations and approved codes of practices.

3.15.2 Electrical supply shall be designed by competent electrician and shall only use
approved type of equipments.

3.15.3 M&E Engineer/Supervisor shall verify the temporary power supply is safe to
be used before the provision could be put into use.

3.15.4 Portable tools shall be 240V unless otherwise agreed by IJM.

3.15.5 All tungsten-halogen lamps must be fitted with glass guard to the element.
These lamps must be permanently fixed at high level.

3.15.6 All electrical machines that consume the supply beyond 240V shall be
certified safe by IJM.

3.15.7 Electrical fittings for outdoor facilities or temporary power installation, which
may be exposed to wet condition, shall be made of waterproof types.

3.15.8 Extension cord shall be in safe to use condition and the wire shall be of double
insulated type approved by SIRIM.

3.15.9 Jointing of wires shall be avoided. Approved connector shall be used for the
purpose. The use of insulation tape to joint wires shall not be allowed.

3.15.10 Only approved industrial type plug and socket shall be used.

3.15.11 Damaged electrical cord shall be removed from site once identified.

3.15.12 No live electrical wire shall be lying on the floor or in water. They should be
hung at an appropriate height.

3.15.13 Live wire should not be tied to electrical conducting material. Using G.I pipe
railing or scaffold to act as electrical wire post shall be prevented.

3.15.14 Adequate hanging post/hook shall be provided at site to enable workers to


hang electrical cord at a safe level.

3.16 Oxygen/Acetylene/Liquid Gas Cylinders Storage

3.16.1 Liquid Petroleum Gas (LPG) cylinders must not be stored or left in areas
below ground level overnight.

3.16.2 The quantity of Oxygen, Acetylene and Liquid Petroleum cylinders at point of
work shall be restricted to a maximum of 1 day’s supply.

3.16.3 Compressed gas cylinders being stored, transported or used shall be in upright
position and secured to some substantial object or structure by a chain or other
suitable means capable of supporting the weight of the cylinder whether
empty or full or be kept in a cylinder rack or cage.

3.16.4 Cylinder valves shall be closed and the hoses pressurized when the cylinders
are not in use. Cylinders shall have protective valve caps in place while they
are being transported and whenever they are not connected for use.

3.16.5 All cylinders shall be returned promptly to a suitable storage area after use.
They shall not be left around the work site.

3.16.6 Oxygen and acetylene tanks shall be stored separately with a safe distance of
at least 6 meter.

3.16.7 Cylinders storage area should be away from general traffic paths and shall not
be adjacent to vehicle paths. Flammable gases and oxidizers shall not be
stored in close proximity unless separated by a non-combustible wall.

3.16.8 Cylinders shall be transported and handled in accordance with all Government
regulations currently in force. Cylinders may only be hoisted when enclosed
in an approved box or cradle within which they are securely fastened. When
the box or cradle is suitably designed, up to two cylinders may be lifted at one
time.

3.17 Plant, Machinery, Equipment and Hand Tools

General Provisions

3.17.1 Use of machine banned by the law such as “monkey hoist” shall be prohibited.

3.17.2 Machinery and equipment with sign of defect or unsafe for use shall not be
allowed to enter the site.

3.17.3 Machinery and equipment shall be operated by qualified trained personnel.


Machinery requiring competent operator shall only be operated by operator
with valid certificate of competency.

3.17.4 Maintenance of machinery shall be carried out by designated personnel only.

3.17.5 Frequent inspection shall be carried out by designated person and any
machine with sign of defects shall be removed from the work area.

3.17.6 Plant, machinery and equipment, including hand tools, both manual and
power-driven, shall:
(a) be of good design and construction, taking into account, as far as
possible, health and safety and ergonomic principles.
(b) be maintained in good working order.
(c) be used only for work for which they have been designed for, unless a
use outside the initial design purpose has been assessed by a competent
person who has concluded that such use is safe.
ad) be operated only by workers who have been authorized and given
appropriate training.
(e) be provided with protective guards, shields, or other devices as
required by national laws or regulations.

3.17.7 Adequate instructions for safe use shall be provided where appropriate by
the manufacturer or the employer, in a form understood by the user.

3.17.8 As far as practicable, safe operating procedures shall be established and used
for all plant, machinery and equipment.

3.17.9 Operators of plant, machinery and equipment shall not be distracted


while work is in progress.

3.17.10 Plant, machinery and equipment shall be switched off when not in use and
isolated before any major adjustment, cleaning or maintenance is done.

3.17.11 Where trailing cables or hose pipes are used they shall be kept as short as
practicable and not allowed to create a safety hazard.

3.17.12 All dangerous moving parts of machinery and equipment shall be enclosed

or adequately guarded in accordance with national laws and regulations.

3.17.13 Every power-driven machine and equipment shall be provided with


adequate means, immediately accessible and readily identifiable to the
operator, of stopping it quickly and preventing it from being started again
inadvertently.

3.17.14 The machinery or equipment shall be designed or fitted with a device where
the maximum safe speed, which should be indicated on it, is not exceeded. If
the speed of the machine is variable, it shall only be possible to start it at the
lowest speed appropriate.

3.17.15 Operators of plant, machinery, equipment and tools shall be provided with
personal protective equipment including, where necessary, suitable hearing
protection.

Hand Tools

3.17.16 Hand tools shall be tempered, dressed and repaired by competent persons.

3.17.17 The cutting edges of cutting tools shall be kept sharp.

3.17.18 Heads of hammers and other shock tools shall be dressed or ground to a
suitable radius on the edge as soon as they crack or fungus begins to grow.

3.17.19 When not in use and while being carried or transported, sharp tools shall be
kept in sheaths, shields, chests or other suitable containers.

3.17.20 Only insulated or non-conducting tools shall be used on or near live


electrical installations if there is any risk of electrical shock.

3.17.21 Only non-sparking tools shall be used in or near the presence of flammable or
explosive dusts or vapours.

Pneumatic Tools

3.17.22 Operating triggers on portable pneumatic tools shall be:


(a) placed as to minimise the risk of accidental starting of the machine;
(b) arranged as to close the air inlet valve automatically when the pressure
of the operator's hand is removed.

3.17.23 Hose and hose connections for compressed-air supply to portable pneumatic
tools shall be:
(a) designed for the pressure and service for which they are intended;
(b) fastened securely to the pipe outlet and a p p r o p r i a t e l y
equipped with a safety chain.

prevent dies and tools from being accidentally expelled from the barrel.

3.17.24 Pneumatic tools shall be disconnected from power and the pressure in hose
lines released before any adjustments or repairs are made.

Cartridge-operated Tools

3.17.25 Whenever practicable, a low-velocity tool shall be used.

3.17.26 Cartridge-operated tools shall have:

(a) a guard or protective shield that is impossible to remove without


rendering the tool inoperative.
(b) a device that prevents the tool from firing inadvertently, for
example if it is dropped or while it is being loaded.
(c) a device that prevents the tool from firing if it is not approximately
perpendicular to the working surface.
(d) a device that prevents the tool from firing if the muzzle is not pressed
against the working surface.

3.17.27 The recoil of a cartridge-operated tool shall not be capable of injuring the
user.

3.17.28 The noise of the detonation shall not able to damage hearing.

3.17.29 A cartridge-operated tool, before each occasion of use, shall be


inspected to ensure that it is safe for use, and in particular:
(a) that the safety devices are in proper working order.
(b) that the tool is clean.
(c) that all moving parts work easily.
(d) that the barrel is unobstructed.

3.17.30 At intervals recommended by the manufacturer the tool shall be


completely dismantled and inspected for use by a competent person.

3.17.31 Cartridge-operated tools shall only be repaired by the manufacturer or


competent person.

3.17.32 Cartridges shall not be stored nor cartridge tools operated:

(a) in a place or environment where these tools could explode accidentally.


(b) in an explosive atmosphere.

3.17.33 When not required for use, inspection or other purpose, cartridge-operated
tools shall be kept in a suitable container that:
(a) is made of suitable material.
(b) is clearly marked to indicate its contents.

(c) is kept locked when not in use.


(d) contains nothing except the tools and cartridges.

3.17.33 No cartridge-operated tool shall be stored or transported in loaded condition,


or left loaded when not in use.

3.17.34 Cartridge-operated tools shall be accompanied by instructions for their


maintenance and use. These tools should only be operated by persons trained
in their safe use.

Electrical Tools

3.17.35 Portable electrical tools shall generally be used on reduced voltage to


avoid as far as possible the risk of a lethal shock.

3.17.36 All electrical tools shall be earthed, unless they are "all insulated" or
"double insulated" tools which do not require earthing. Earthing shall be
incorporated in metallic cases, which acts as a safeguard against damaged
cables where wires enter the tool.

3.17.37 All electrical tools shall be inspected and maintained on a regular basis by a
competent electrician, and complete records shall be kept.

Woodworking Machine

3.17.38 Shavings, sawdust, etc, shall not be removed by hand from woodworking
machines or in their vicinity while the machines are working.

3.17.39 Where provided, chip and sawdust extraction systems shall be maintained in
efficient working order.

3.17.40 Mechanical feeding devices shall be used whenever practicable.

3.17.41 All cutters and saw blades shall be enclosed as far as practicable.

3.17.42 Circular saws shall be provided with strong, rigid and easily adjustable hood
guards for the saw blades and with riving knives of suitable design matched to
the saw blade in use. The width of the opening in the table for the saw blade
shall be as small as practicable.

3.17.43 Portable circular saws shall be designed that when the blade is running
idle it is automatically covered.

3.17.44 Woodworking machines shall be properly spaced to avoid accidental injury


when handling large boards or long planks.

3.18 Scaffolds

3.18.1 Scaffolds shall be erected in accordance to Part X of Factories And Machinery


(Building Operations And Works Of Engineering Construction) (Safety)
Regulations 1986 and applicable standards.

3.18.2 All materials to be used for the construction of scaffolds shall be inspected by
a designated person.

3.18.3 All scaffold component must be in good condition and free from corrosion or
other patent defect and suitable for the task to be performed.

3.18.4 Defective scaffold component shall be segregated or removed from site.

3.18.5 Erection, alteration or dismantling of scaffold, where the height is more than 4
metres must be supervised by a competent person.

3.18.6 Pipe and tubular scaffold exceeding 40 metres in height and frame scaffold
exceeding 15 metres in height shall be erected in accordance with the
endorsed design and drawings of a Professional Engineer.

3.18.7 Foundation for scaffold base must be firm and able to carry the whole weight
of the scaffold. Sole plate shall be used if the ground is not hard enough and
there is potential of penetration.

3.18.8 Extra scaffold component shall not be left on erected scaffold to prevent
hazard of falling object.

3.18.9 Competent person shall inspect the erected scaffold once every 7 days as well
as after every exposure to adverse weather conditions in which the inspection
shall be recorded.

3.18.10 Scaffold Green Tag shall be affixed at the scaffold by competent person once
it is classified as safe to be used.

3.18.11 In any case where the scaffold is found not safe to be used, competent person
shall affix Scaffold Red Tag at the scaffold.

3.18.12 Scaffold under construction or dismantling shall not be allowed to be used. A


prominent warning notice in the national language apart from Scaffold Red
Tag indicating that the scaffold is prohibited for use shall be affixed on the
scaffold.

3.19 Edge Protection And Floor Opening Protection

3.19.1 All work areas with exposed edges from which a fall of more than 3 metres is
possible shall be provided with edge protection consisting of guardrails, mid-
rails and toe board.

3.19.2 Guardrail shall be between 0.9m and 1.1 m above working floor/platform
level. Mid-rail shall not be less than 690 mm between guardrail and working
floor/level. Toe-board shall not be less than 200mm.

3.19.3 Guardrails and mid-rails shall be capable of sustaining, without failure or


undue deflection, a force at any point of .69kN (70kg) vertically and .44kN
(45kg) horizontally.

3.19.4 Guardrails and mid-rails shall use material such as G.I pipes or equivalent.
Use of warning plastic tapes of red/white stripped in colour as protection
against falling shall be prohibited.

3.19.5 Guardrails and mid-rails shall be supported and maintained at their required
position by vertical guardrail post.

3.19.6 Guardrails post shall be secured to completed floor using approved anchorage
method such as drop in anchor bolt or equivalent.

3.19.7 Guardrail post at area where slab formwork installation is on going shall be
securely fixed to temporary structure using approved method.

3.19.8 Provision of edge protections shall be maintained in good condition at all


times.

3.19.9 Whenever installation of edge protections is not practical due to unavoidable


limitation in the work process, lifeline shall be provided at exposed edges
using material of adequate strength to serve the purpose of installation.

3.19.10 All large floor openings with the potential for a person to fall through shall be
provided with protection similar to exposed edges.

3.19.11 Small hole without the potential for a person to fall through shall be covered
with a secured cover, built to the standard of a light duty platform or such
greater load could be imposed on it.

3.19.12 Proper planning shall be done before installation to avoid obstruction to work
activities at/near exposed edges such as brickworks, cladding and others
activities.

3.19.13 Any need to dismantle any protection to allow for works installation at any
time must be approved by the authorized person.

3.20 Ladder And Step Ladder

3.20.1 All ladders shall comply with the relevant Malaysian Standard or other
accepted International Standard.

3.20.2 Use of timber ladders shall be regarded as substandard and prohibited.

3.20.3 Ladder and stepladder to be used at site shall be of industrial type. Using
domestic type ladder and stepladder shall be avoided.

3.20.4 Single pole or extension ladder used for access shall be secured against
movement or sliding at top and bottom while in use.

3.20.5 Ladder must extend at least one metre above landing/work platform to ensure
adequate handhold.

3.20.6 The ratio of ladder height and base shall be 1:4 when positioned at site.

3.20.7 Work to be carried out on ladder and stepladder shall be limited to light and
short duration works.
3.20.8 Ladder and stepladder shall be set up on level and firm ground only.

3.20.9 Stepladder shall be fully opened to ensure stability.

3.20.10 No work shall be carried out on the top three steps of any stepladder.

3.20.11 Side-on working shall be avoided.

3.20.12 Ladder shall be prohibited for use and immediately removed on suspicion of
any structural damage.

3.21 Elevated Working Platform

3.21.1 Working platform for elevated activities shall strictly comply with applicable
legal requirements and code of practice.

3.21.2 Working platform from which a person is liable to fall more than 3 metres
shall have a minimum width of at least;

a. 635mm wide for platform being used only as a footing and not for
deposit of any material.
b. 860mm wide if the platform is used for the deposit of material.
c. 1.10m wide if platform is used for the support of any higher platform.

3.21.3 Working platform shall be provided with edge protection railings and toe
board. Ladder shall be provided for safe access.

3.21.4 Working platform shall have no gap of which materials could fall onto person
below.

3.21.5 Only approved material such as metal catwalks, boards or planks shall be used
as a working platform.

3.21.6 Boards or planks shall have sufficient thickness based on the distance of
support underneath them;

a. Board with 32mm thickness shall be supported at every 1.0m.


b. Board with 38mm thickness shall be supported at every 1.50m
c. Board with 50mm thickness shall be supported at every 2.60m

3.21.7 Board or plank shall not be less than 200mm wide or in the case of its
thickness exceeding 50mm, not less than 150mm.

3.21.8 Board or plank shall not extend beyond its end support to a distance exceeding
four times of its thickness unless it is effectively secured to prevent tipping.

3.21.9 Inspection shall be carried out on working platform by work supervisor to


ensure safe condition at all times.

3.22 Concrete Reinforcement Starter Bars

3.22.1 All protruding concrete reinforcement starter bars either vertically or


horizontally positioned with the potential of puncturing hazard must be
protected either by hooked starters, plastic protection caps, plywood covers or
other approved methods.

3.23 Falling /Flying Objects

3.23.1 All workers working at the perimeter of the building / structure or around
openings within the structure shall have their hand tools secured to them by
means of lanyard.

3.23.2 When stacking or storing loose material at height, a distance of 3m from any
exposed edge shall be maintained to prevent falling of material to lower level.

3.23.3 No loose material shall be kept on scaffold unless safely secured against
falling.

3.23.4 Plywood stacking at height shall be secured to avoid them from falling due to
strong wind.

3.23.5 Lightweight construction material shall be arranged and positioned in a


manner that it will not be blown away in case of strong wind.

3.23.6 Cleaning of formwork to receive concrete using air compressor shall be done
in a manner that debris will not fly down to lower level.

3.23.7 Area where overhead activity is carried out shall be cordoned off to prevent
entry. A warning notice in the national language shall be posted to indicate the
overhead work hazards.
3.23.8 Working yard at area where there are potential risks of being struck by falling
objects shall be provided with adequate strength overhead protection to
protect personnel working at those areas.

3.24 Personal Protective Equipments

3.24.1 All personal protective equipments to be provided shall conform to national or


acceptable international standards.

3.24.2 Wearing of safety helmet and shoe shall start at workplace’s entrance, where
security personnel shall check the compliance.

3.24.3 All site employees shall wear safety helmets and safety foot wear at all times
whilst at site. The wearing of sandals, flip-flops and trainer type footwear is
strictly prohibited.

3.24.4 Workers must wear safety harness when working at height with the potential
of fall of more than 3 meters in the event a safe working platform with
adequate fall protection is not practical to be provided.

3.24.5 Safety belt shall be used for travel restriction purpose only, where it shall be
rigged in such a way that it will stop the worker from reaching a position
where a free fall is possible.

3.24.6 Workers shall be provided with eye protection when carrying out activity with
potential eye injury.

3.24.7 Suitable glove designed for specific protection against work risks shall be
provided for worker exposed to potential hand injury activity.

3.24.8 Ear protection shall be provided to workers exposed to activity with potential
hearing impact.

3.24.9 All PPE issued to workers must be recorded and submitted to IJM for filing.

3.24.10 Workers shall be trained on the correct use, storage and maintenance of PPE.

3.24.11 Damaged PPE shall be replaced immediately once detected.

3.24.12 Inspection by site supervisor shall cover PPE compliance.

3.25 Hazardous Chemical Management

3.25.1 “Hazardous Chemical” means any chemical which posses the nature of risk to
safety, health and environment such as:

 Explosive
 Oxidising
 Extremely flammable
 Very toxic
 Corrosive
 Harmful
 Irritant

3.25.2 Any hazardous chemical in use on site must be in compliance with the
Occupational Safety and Health (Classification, Packaging and Labelling of
Hazardous Chemicals) Regulations 1997 and Occupational Safety and Health
(Use and Standards of Exposure of Chemicals Hazardous to Health)
Regulations 2000.

3.25.3 Packaging and Labelling

3.25.3.1 When entering the site, all hazardous chemical shall fulfil the packaging and
labelling requirement as follows:

 The container shall be designed in a manner that can keep the chemical
from being exposed to the atmosphere. The material used for the
chemical container must not be liable to form any harmful or
dangerous compounds.
 Secondary container should be relabelled as per original labelling. The
secondary container must be clean and free from other compounds
before refilling it with chemical from the original container.
 Use of drinking water bottle as secondary container is strictly
prohibited at site.
 All hazardous chemical in use at site must be labelled clearly. The
label must state:
a) Chemical name as stated in Chemical Safety Data Sheet or
international nomenclature.
b) Warning signage for related chemical hazards.
c) The manufacturer’s information (name and address)

3.25.4 Handling

3.25.4.1 Chemical shall be handled according to the specifications detailed in the


Material Safety Data Sheet.

3.25.4.2 Hazardous chemical shall be handled safely with the use of approved
personal protective equipments relevant with the degree of exposure to
health and body.

3.25.4.3 The chemical shall only be handled by trained personnel.

3.25.4.4 The exposure of any person to any hazardous chemicals must be in


compliance with permissible exposure limit (PEL) and shall not exceed the
maximum exposure limit in any eight hour work shift.

3.25.4.5 Removal and disposal of chemical waste shall be carried out in safe and
scheduled system.

3.25.5 Chemical Storage

3.25.5.1 All hazardous chemical shall be stored in accordance to the specifications in


Material Safety Data Sheet.

3.25.5.2 Designated area with adequate warning signs and fire extinguisher must be
made available for chemical storage purposes.

3.25.5.3 “Designated area” means special storage for chemicals where:

 It is isolated from the workers’ environment or public access.


 It is enclosed or shielded with physical barriers with openings or
windows.
 It is placed on concrete or brick cradles.
 The store shall be locked to prevent unauthorized entry.

3.25.5.4 For the storage of liquid chemical such as diesel and petrol, the following
points shall be noted:

 It shall be stored in a designated area.


 The storage tank shall be made of mild steel as well as covered or
fastened at all times.
 Oil tight catch pit or bund with the capability to contain the worst
spillage condition ( i.e. 110% of the storage capacity of the largest
container) shall be provided.

3.25.6 Monitoring

3.25.6.1 All hazardous chemicals shall be identified and recorded in the chemical
register. The register shall be maintained in good order and contain the
following information:

 List of hazardous chemical being used on-site.


 The current Chemical Safety Data Sheet for each hazardous chemical.
 The process and work area where hazardous chemicals are being used.

 The authorized personnel for handling of hazardous chemicals.

3.26 Passenger and Material Hoist

3.26.1 All passenger and material hoist component shall be inspected and approved
by DOSH prior to erection on site.

3.26.2 No passenger and material hoist is allowed to be used at the workplace


without the proper and valid certificate of fitness (PMA).

3.26.3 Passenger and material hoist must be erected by competent person approved
by DOSH.

3.26.4 Only trained operators shall operate the hoist.

3.26.5 Only fully enclosed passenger and material hoist with falling object protection
structure can be used at the workplace.

3.26.6 Sufficient OSH sign, Safe Working Load (SWL), PMA number, Person in
Charge contact number, and Safe Work Instruction of passenger and material
hoist shall be displayed.

3.26.7 Gate/door of sound construction with adequate railings for every landing floor
at the building structure shall be provided. The gate/door shall be opened and
closed only by the hoist operator.

3.26.8 Any repair work especially on critical parts of the hoisting machine shall
follow the manufacturer’s specifications. The repair work shall require
approval from the Person in Charge of the project.

3.26.9 Maintenance and repair work shall only be carried out by approved competent
person.

3.26.10 Whenever the hoist is suspected of being defective, its operation shall be
immediately stopped and shall not resume until it is inspected or repaired by
competent person.

3.26.11 Maintenance and repair shall only be carried out by approved competent
person.

3.26.12 The owner of passenger and material hoist shall ensure that all safety devices
(e.g. overload limit switch, safety gear, power failure, over-run switches,
emergency button devices and brakes) are maintained in good working
condition and periodically tested and serviced.

3.26.13 Loading of passenger and material hoist shall not exceed the SWL at all times.

3.26.14 Preventive maintenance and inspection shall be conducted regularly to ensure


passenger and material hoist is in good working condition at all times.

3.26.15 The hoist shall be secured after work by ensuring:


- The hoist is fully rested at the lower ground level
- Cage interior cleanliness.
- Power supply system is shut down completely
- All gates are locked to prevent unauthorized access.
