AUTOPSY REPORT PATHOLOGICAL EXAMINATION ON THE BODY OF DENNIS HOF AUTOPSY FINDINGS Atherosclerotic and hypertensive cardiovascular disease: A. 3-vessel coronary artery atherosclerosis, severe, with recent thrombosis of right coronary artery. 1, Acute myocardial infarction, left ventricle, posterolateral wall, and interventricular septum, posterior. 2. Remote myocardial infarctions, left ventricle, anterolateral and posterolateral walls, and interventricular septum. B. Aortic atherosclerosis, severe. C. Cerebrovascular atherosclerosis, severe. D. Cardiomegaly (heart weight: 650 grams) with left ventricular hypertrophy. E. Lacunar infarcts, pons. F. Nephrosclerosis. Diabetes mellitus (per history): A. Nephrosclerosis. B. Pancreatic fibrosis and fatty infiltration. III. Obesity (Body Mass Index 32.7): Iv. v. vr. A. Hepatic steatosis. Chronic obstructive pulmonary disease, mild. Renal mass, left kidney, histologically suggestive of renal cell carcinoma. Horseshoe kidney. VII. Splenomegaly (spleen weight: 470 grams). VIII.Cholelithiasis. Ix. x. xr. Toxicology: A, Delta-9 THC (active ingredient of marijuana) and its metabolites 11-Hydroxy Delta-9 THC and Delta-9 Carboxy THC detected. Sildenafil (110 ng/mL) and its metabolite N- Desmethylsildenafil (35 ng/ml) detected. C. Non-toxic levels of arsenic (6.4 mcg/L), selenium (200 meg/L), and lead (3.4 meg/dL) detected. D. Caffeine detected. E. Gamma-hydroxybutyric acid, cyanide, oleandrin, and swainsonine NOT detected. F, Vitreous chemistry non-contributory. G. See separate toxicology reports. OPINION According to the investigative information provided, this 72- year-old man was celebrating his birthday with friends on the night of October 15, 2018. He returned to a local brothel (which he owned) with friends at approximately 2300 hours and went into a bedroom with a female employee. She left the room at 0050 hours and went to her bedroom. When he had not emerged from the bedroom or responded to text messages by 1030 hours, the woman asked a male friend to check on him. The woman, the male friend, and the house manager entered the room and discovered him lying supine on the bed and unresponsive. 911 was called. Emergency medical services found him beyond resuscitation, and death was pronounced at 1100 hours. The autopsy documented an obese Caucasian male with the findings listed above, which included a recent thrombus in the right coronary artery and an acute myocardial infarction involving the posterior wall of the left ventricle and interventricular septum. The microscopic features are consistent with the infarction occurring between 4 to 12 hours prior to death. No significant traumatic injuries were identified. Toxicological testing detected Delta-9 THC (active ingredient of marijuana) and its metabolites 11-Hydroxy Delta-9 THC and Delta- 9 Carboxy THC, therapeutic/non-toxic levels of sildenafil and its metabolite N-desmethylsildenafil, and caffeine.
