DAILY SAFETY TOPIC: Broken Ladder

April 25, 2018 – A coworker set up a ladder which had the swing arm piece broken
off on one side; the ladder was being held together with a rope to prevent it from
overextending or twisting under the worker’s weight and movement. The employee
stopped the coworker and explained there are plenty of ladders around and this
was not safe. The employee grabbed a ladder in good condition and tagged the
broken one to be fixed; thus, eliminating any risk of injury.
May 2, 2018 (pictured below) – An employee noticed a ladder which was completely
broken and had rope tied on it which does not help at all. This is very dangerous
and would cause a serious incident which may result in severe injuries. The ladder
was immediately removed from service. (Thank you to the employee who took the picture.)
There is no way to tell if this is the same ladder as the April 25th observation as the
first one was tagged out.
IMPORTANT: Equipment, tools, ladders, etc. which have been tagged out MUST
NOT BE put back into service until they are repaired.
