We, JOHN MICHAEL and CHRISTINE, both of legal age, single, Filipinos, and
residents of _______________________, after having been duly sworn to in accordance with
law hereby voluntarily depose and state:

That we are the biological parents of minor CHRISTIAN who was born on

That our child will travel to Hongkong with his maternal grandfather

That the said child will be leaving on _________________ to Hongkong for a

summer vacation and will stay there for a period of _____________;
