Good day!
We, fourth year students of BS Industrial Engineering, are currently carrying out a study
entitled “Application of Maslow’s Theory in the Assessment of Academic Performance of
Selected Industrial Engineering Students of Cavite State University-Main Campus”. The
study is focused on sanitation and accommodation and how such affects their academic
performance. We therefore kindly request you to freely respond to all questions in the study
questionnaire. All data collected will be treated with high level of utmost confidentiality and will
only be used for research purposes.
