Instructions: Using the OnHand Schools EdInsight System, look at the data for each of your classes, and complete the
following information. You may type directly in the boxes.

What are the strengths of the class?

Students in this class are not demonstrating any real significant strengths in the area of Reading as measured by either
the PSSA or the 4-Sight Benchmark assessments. Five (5) students (Pirelli, Boxar, Manley, Lackton and Shelly) scored
proficient on the PSSA. These students show some strengths in their score levels on the benchmark assessments in the
areas of vocabulary.

There is some data from the current 4-Sight assessment that demonstrates that 10 of the students have some skill in
character analysis with scores in 60% range, but those scores are not supported with other strengths.

What are the weaknesses of the class?

22 of the 27 students in the class scored Basic or below on last year's PSSA and those same 22 students have been
projected to be basic or below this year based on the current 4-Sight assessment.

Within those 22 students, several students (Buchannon, Kovalich, Nelson, Johnson, Michaels, Carrodi, Branniff and
Tinstall) have registered correct response percentages below 40% on all of the eligible content that addresses
comprehension in both fiction and non-fiction. The remaining students within that group are not much better averaging
just over 50% correct responses in those critical areas.

What is/are your improvement goal(s) for the class?

To raise student achievement scores levels in comprehension strategies for both fiction and non-fiction to proficient or
above by the next benchmark assessment.

Specific focus placed on the 17 students who are currently below proficiency and the five students who are below basic
in most of the primary skill sets that underpin comprehension.

To produce an organized and systemic achievement monitoring system for struggling readers through integration of
classroom instruction, focused interventions and increased communication with students and parents.

How will these goals be measured?

The target goals will be measured by achievement levels on the 4-Sight benchmark assessment. The target score levels
are 80% correct in all eligible content that addresses comprehension including:

The Action Plan : Briefly describe your plan to accomplish the goal.

The first step will be to review all the Reading achievement data on the 20 students who are currently below proficiency
to obtain lexile reading scores and reading levels.

With this data, the class will be divided into reading groups. Supplemental reading material leveled for each group will be
used to focus each reading group on working with comprehension strategies with text levels they can read.

Each day the reading groups will work with the teacher one group at a time. While one group is working with the teacher
the other groups will be working at learning centers designed to focus on their specific needs. Each learning center will
be designed to meet needs identified in the benchmark data.

Each day students will be responsible for completing work in the learning centers. This work will reflect their level of need
and will be used as a scaffold to guide student learning. The work will be collected in student portfolios that will become
part of each student's grade.
