1.0 PURPOSE:
1.1 Ensure a rapid response to and recovery from HSE incidents.
1.2 Ensure proper reporting.
1.3 Prevent injury to public and PGCC personnel (as well as all interested
parties).
1.4 Minimize damage to property and the environment.
1.5 Comply with legal requirements.
1.6 Determines the cause(s) with a view to preventing a recurrence.
1.7 Gathers information for use in any criminal or civil proceedings.
1.8 Confirms or refute a claim for compensation benefit.

2.0 SCOPE:
This procedure applies to the following:
2.1 All HSE incidents / Accidents that may happen at PGCC Sites & Offices.

3.0 RESPONSIBILITIES:

3.4 HSE Officer
3.5 Accident discoverer /witness .
3.6 Supervisor/Foreman /Emergency In charge

4.0 PROCEDURE INPUT :


4.1 HSE Procedures (in general).
4.2 Emergency plan.
4.3 Hazard/aspects identification, risk/impacts assessment.
4.4 HSE Internal audit reports, including non-conformance reports.
4.5 Accident, incident and/or hazard/aspects reports.
4.6 Maintenance and service reports.

5.0 PROCEDURE OUTPUT :


5.1 Accidents/incidents & non-conformance reports;
5.2 Investigation reports;
5.3 Updated Hazard/aspects identification, risk/impacts assessment;
5.4 Management review input;
5.5 Evidence of evaluations of the effectiveness of corrective and preventive
actions taken.

6.0 PROCEDURE:

6.1 Accidents Classification**


PGCC has classified the accidents into four main categories as follows:-
Sr. Classification Details

5.1.1 Major Injury : An injury arising out of, or in connection with work
which results in any of the following:-
- Fracture of the skull, spine, pelvis or any bone in
the wrist, arm, leg or ankle.
-Amputation of a hand, arm, foot, finger, thumb or
toe.
-Loss of consciousness due to lack of oxygen.
-Absorption of any substance by inhalation,
ingestion, or through the skin.
-Loss of consciousness from an Electric shock.
Loss of sight of an eye or a penetrating injury,
chemical or hot burn.
-Death of any person.
-Any other injury that results in the person injured
Being admitted to Hospital for more than 24 hrs and

5.1.2 Lost Time (Reportable) Injury arising out of, or in connection with work which
Injury: involves a person being absent from work for more
than two days. (Excluding day of accident/injury).

5.1.3 Minor Injury: Injury arising out of, or in connection with work which
does not fit into any of the above categories.

5.1.4 Incidents (Near Miss ): Unplanned and unwanted event which disrupts the
work process and has the potential of resulting in
injury, harm, or damage to persons or property.

5.2 Accidents Handling & Reporting ( Major & Loss Time Injury)
