Petitioner, through the undersigned counsel, to this Honorable Court

respectfully states, that:

PETITIONER, Al Capone, is a Filipino citizen, of legal age, single, and is a

resident of 976 Osmena Drive, Fuente, Cebu City, Cebu, Philippines, and may
be served with copies of pleadings as well as of writs, notices and other court
processes of the Honorable Court through his counsel with office address at
2-F, Melecio Tan Building, Trese Martinez St. Cebu City, Cebu, Philippines;

Johnny Torrio, (hereinafter referred to as WARD). for whom letters of

guardianship are prayed to be issued, is a Filipino citizen, of legal age, single,
and is a resident of 123 ABC Bldg, Fuente Osmena, Cebu City, Cebu,
Philippines where he may be served with summons notices, writs and other
court processes;

WHEREFORE, PETITIONER, respectfully prays, that:

1. this Honorable Court fix a time and place for the hearing of this
Petition;
2. due notice be given to all persons who under the Rules of Court are
entitled there to; and
3. after due hearing and filing of a bond the amount as the Court may
determine, Letters of Guardianship over the person and property of
WARD be issued to PETITIONER.
