1. Description of activities you engaged in this period.


-Training on the DCS system.
-Avoiding any errors during the uploading process.
-Finishing on the cardiac strip scanning.
-Start to index what I scanned.

a) Describe your activities in some detail.


Some of the activities that I performed during this period of time include the retraining
on the DCS system in order to make sure that I knew all the steps and avoid any errors during
the process of my work. After that, I started to scan the rest of the cardiac strips in order to
prepare myself for indexing those scans. Once I finished with the scanner, I proceeded to work
on my desk to index the cardiac strips in which I received another training from my supervisor.
Ms. Squadrito taught me how to index by checking the images that I had scanned with the
papers that I had in my desk to make sure that everything matched accurately and adequately.
If one of the papers did not match, then I would put the paper on the side of my desk to later
rescan the missed document and finally ensure that it was matched correctly. If everything was
right, then the next step was the division of the cardiac strips by patient and the patients' IDs
which, in my case, the patients' IDs were composed of 9 digits.

b) Include your reactions to these activities and to your clients and peers.
My reactions were very professional, but internally I was so happy for all the process that
I was watching and involved in; it was very exciting to actually see what the real world of my
major was like. This week was more of getting to know the DCS program and understand the
staff’s daily tasks to prepare for what I was going to be working with and the work I was going
to be looking at.
c) Were you supervised during these activities?
Yes, I was supervised during these activities. The majority of the time Ms. Squadrito was
with me, but if I was in an employee’s office or in my office, then I was without supervision.

1. If yes, describe the supervision and include your reaction to your supervisor
and supervision.
The supervision was very professional, and, at the same time, I felt their
supervision was great since they always made me feel so welcome. My reaction to
Ms. Squadrito supervision was professional and very friendly and always made
sure I was understanding everything.

2. If no, describe your activities when you were not supervised and your
reactions.
When I was without supervision, I had the same behavior as if I was being
supervised because I wanted to make sure everything I did was done effectively
and responsibly. I had a great respect for Ms. Filipovich and it was never my
intention to lose the trust and faith that she gave to me.

2. What did you expect to learn through these activities?

I expect to learn more about the usage of the DCS system and the office’s daily
work, purpose and Mission, are true according to the actions that the staff perform
and the services that they provide.

I did not have any direct contact with the clients.

3. What exactly did you learn through these activities?

I learned about the usage of the DCS system and how to avoid errors during the
uploading process of the patients’ information. In addition, I learned the scanning,
and indexing of these documents.

4. Can you identify any initiatives as required by the organization’s performance improvement
planning or as required by the regulatory bodies, such as CMS?
a) If so, what are they and who is requiring them?
Yes, I can. Ms. Filipovich office is one of the offices that was in charge of
regarding some of the obligations as an intern; I was in charge of manage and deliver
patients medical information. I helped Ms. Filipovich in these activities in order to
improve the performance plan of the information management of Crouse’s patients and
help the team to keep the medical information up to date and organized in a more
effective way.

b) If you can't identify do you see an opportunity for performance improvement?

5. What activities do you expect to be engaged in during the next 40 hours of internship work?
I am expected to be engaged in the right usage of the DCS system without any mistakes.

6. What do you expect to learn during the activities of the next 40 hours of internship work?
I am expected to learn how to use the DCS system in the quickly way possible with the
minimum error possible.

7. What do you WANT to learn? Have you communicated this to your supervisor?
I want to learn how to avoid errors during the process of upload the patients documents to
the computer system. Yes, I have communicated this to my supervisor.
