AFFIDAVIT OF NO PENDING CASE

I, ROSEANNE RECAMARA y SONIO, Filipino, 24 years old, single and a

resident of Brgy. Palale, Sta. Margarita, Samar, after having been duly sworn to law,
hereby depose and state:

1. That I am intending to apply as CANDIDATE SOLDIER in the Armed

Forces of the Philippines, Philippine Army, and when I inquire about the
necessary documents to facilitate my application, one of the
requirements is an affidavit of no pending case;

2. That I have no existing criminal and civil cases based on the records duly
issued by the Office of the Clerk of Court in Calbayog City and in effect
was issued a Court Clearance for the same purpose;

3. That I am executing this to attest to the truthfulness and veracity of the

foregoing statement and likewise for whatever legal purpose this
instrument may serve;
