SETTLEMENT AGREEMENT

On June 10, 2019, the Bureau of Community and Health Systems issued an Order of
Summary Suspension and Notice of Intent to Revoke the child care center license of HKSL,
Le.
|n settlement of all issues raised in the Order of Summary Suspension and Notice of
Intent to Revoke, the undersigned parties hereby agree:
1) HKSL, LLC does not contest the allegations and violations set forth in the Order of
Summary Suspension and Notice of Intent to Revoke, and the Bureau may treat the
aliegations of fact and law as true, which finding shall have the same force and effect
for purposes of this settlement agreement as if evidence and argument were
Presented at an administrative hearing in support of the allegations.
2) HKSL, LLC understands that its child care center license will be revoked, effective the
date of the signature by the representative of the Bureau of Community and Health
Systems. HKSL, LLC agrees to immediately cease providing cate to children at the
center.
3) HKSL, LLC and Licensee Designee Deanna Gleason understand that they are waiving
their right to appeal the Order of Summary Suspension and Notice of Intent to Revoke
the license; to attend a compliance conference; and to request an administrative
hearing in this matter,
4) HKSL, LLC and Licensee Designee Deanna Gleason understand that pursuant to
MCL 722.125(4), the department will reject any application on its face submitted by
the HIKSL, LLC and/or Licensee Designee Deanna Gleason in the five years following
the effective date of this Agreement.
5) HKSL, LLC and Licensee Deanna Gleason understand that pursuant to MCL
722.125(4), both are prohibited from being connected, directly or indirectly, with a
licensed child care home or center for a period of five years aiter the effective date of
this Agreement.

722.125, Section 15(1), a person who provides child care without a license or
certificate of registration is gullly of a misdemeanor, and the offense is punishable by
up to 90 days imprisonment and/or a $1,000 fine.
