STUDENT DISCLOSURE CONTRACT

_______________________, after having been duly sworn in accordance with law, hereby depose and state that:

(*hereinafter referred to as INSTITUTION); or

______ I AM A MEMBER of ____________________ fraternity, sorority or organization, but attached is a written undertaking to quit, to
resign or to renounce my membership from said fraternity/sorority, organization not recognized by the INSTITUTION.

2. I have accomplished this undertaking with my personal knowledge and I am submitting voluntarily to the guidelines set by the
INSTITUTION.

3. That during my entire stay at Emilio Aguinaldo College, I SHALL NOT JOIN any fraternity, sorority or organization that is not
recognized by the INSTITUTION;

4. I acknowledge and understand that my admission in Emilio Aguinaldo College is a privilege and not a right and that the
INSTITUTION has the sole authority to choose the persons or individuals that may be admitted as its students.

5. I acknowledge and understand that the INSTITUTION has the sole authority to prescribe rules and regulations governing non-
membership of students in any fraternity, sorority, or organization in EAC and that these rules are valid because they are based on
the constitutional right of schools of higher learning to prescribe the conditions they may require of any person or individual
aspiring to become their student. In addition public policy provides for the following:

5.a That the Government recognizes of the right of schools of higher education such as the INSTITUTION to withhold
acknowledgment to fraternities, sororities or organizations considered detrimental to peace and harmony within the school
campus.

5.b That the existence of groups such as fraternities and sororities in EAC has the potential to disturb the unity and
peace which the INSTITUTION wishes to foster among the members of its academic community.

5.c That the presence of fraternities, sororities or organizations that are not recognized by EAC has contributed to the
occurrence of violent skirmishes and isolated incidents of student-directed crime on campus in the past.

5.d That membership in fraternities, sororities or organizations that are not recognized by EAC has developed
undesirable traits and character among its student population which is contrary to the institution’s MISSION STATEMENT.

5.e That the absence of fraternities, sororities or organizations in EAC has facilitated the rapid development of
harmonious and peaceful relations among Emilians.

6. I recognize and accept that my continued stay in EAC is subject to my compliance with prescribed discipline policies, rules and
regulations especially with my non-membership in fraternities, sororities or organizations.

7. I am fully aware of the provisions of Law regarding Hazing and other forms of Initiation Rites as stated in R.A. 8049.
I am also aware of the consequences should I violate the said law.

8. I UNDERSTAND AND ACCEPT THAT EMILIO AGUINALDO COLLEGE CAN EITHER DISMISS OR EXPEL ME IF I WOULD BE
FOUND HAVING VIOLATED ANY OF THE CONDITIONS TO THIS AFFIDAVIT AS WELL AS HAVING FALSELY CERTIFIED
ANY INFORMATION STATED HEREIN AFTER DUE PROCESS.

9. I am executing and submitting this affidavit as a prerequisite of my admission as a student of the INSTITUTION.
