ACKNOWLEDGEMENT AND ASSUMPTION OF RISKS and

WAIVER AND RELEASE OF LIABILITY AGREEMENT

In consideration for being allowed to participate in any way in the Arlington Premier Soccer
League, LLC. (APSL, LLC.) soccer games and related events and activities I the undersigned:

1. Agree that prior to participating, I will inspect the fields, facilities and equipment to be
used, and if I believe anything is unsafe, I will immediately advise my team coordinator or
his/her representative of such condition(s) and will refuse to participate;

2. Acknowledge and understand that soccer is a contact sport and that each participant will
be engaging in activities that involve serious injury, including permanent disability, and/or
death and severe social and economic losses which might result from my own or others’
actions, inactions or negligence, the laws of the game, or the condition of the
fields/premises or of any equipment used. Further, that there may be other risks not
known to me or not reasonably foreseeable at this time.

3. Assume all the foregoing risks and accept personal responsibility for the damages
following such injury, permanent disability or death.

4. Release, waive, discharge and covenant not to sue, APSL, LLC., its managers, members,
agents, and any employees of the organization, its affiliated clubs and/or teams, and their
respective coaches, team coordinators/administrators, or sponsoring agencies, sponsors,
advertisers, the owners and leasors of premises used to conduct events, and other
participants or parties including referees (all of which hereinafter referred to as "releasees")
from demands, losses or damages on account of injury, including death or property
damage caused or alleged to be caused in whole or in part by the negligence of the
releasees or otherwise.

5. I, the undersigned, have read the above waiver and release and understand that I am not
covered by any health or injury insurance provided by or through the APSL, LLC. and
that I given up substantial rights by signing this and that I sign it voluntarily, and
knowingly assuming the risk.

6. I have read sections 1 through 5 above and I fully agree to be bound by their provisions. I
further agree that I shall abide by (1) the League rules and regulations as documented in
the APSL, LLC. Membership and Operating Rules (current edition) and (2) the decisions
made by the APSL, LLC. Executive Board. I also confirm that I am 18 years old or older
as of the date of signing this agreement.
