X Today's Date Enter today's date
X Project Name Give the Project a name. This will be the internal, working name and as such, should be short but concisely identify the project.
X Project Summary Include a brief description of the project including the product or service description and what it will replace (if applicable).
X Strategic Goal Pick list - select the IT Services Strategic Goal that this project is aligned with or supports.
X Start Date The date that this project started
X Projected End Date The date that this project is expected to end
X Status Pick list - select the status that best describes where the project is at this point in time (see legend below)
X Business Owner The person/s in the organization that will take ownership of the project's outputs and use them for the benefit of the organization.
X Project Manager Name the person assigned to achieve the project objectives.
X # of Project Team Members The total number of people that are involved in the implementation of this project
X # Months The total number of months that the project team members will participate
X Impacted Groups List the average percentage of their time needed per week.
Comments Optional field. Can be used for any comments you feel are important to note.
