1.0 DEFINITION
BCE: BAKTICON ENGINEERING SDN.BHD.

2.0 INTRODUCTION
BAKTICON ENGINEERING SDN. BHD. considers the health and safety
of its employees, the client’s employees and to members of the public.
We also pledge not to cause any harm to the environment.

The achievement of the aims of this plan will require continued


commitment and sustained efforts by everybody concerned. All
employees have a personal responsibility to prevent injury or risk to
health to themselves and to anyone else who may be affected by their
actions.

BAKTICON ENGINEERING SDN. BHD. required that all the


management, supervisory staff and sub-contractor to read this plan.

The HSE Plan has been developed in accordance with the requirements
of the “Laws of Malaysia”, Act 514, Occupational Safety and Health Act
1994.

3.0 SCOPE
The intention of this document to describe the action plan to manage
all HSE activities to safeguard personnel and equipment during the
work activities. This HSE Plan shall apply equally to all Sub-
Contractors employed by BAKTICON ENGINEERING SDN.BHD

4.0 ORGANIZATION
The BAKTICON ENGINEERING SDN.BHD Management Team shall be
fully responsible for the implementation of the planned work activities.
The Safety Officer shall provide the necessary support for the execution
of the HSE plan.

5.0 HSE OBJECTIVES AND TARGETS


It is BAKTICON ENGINEERING SDN.BHD pledge to demonstrate
respect for the safety of all persons and the minimization of any
environmental impacts in order to achieve our goals of no harm to
people and no damage to the environment; and ZERO
accident/incident. Everybody who works with BAKTICON
ENGINEERING SDN.BHD including sub-contractors is responsible to
carried out work in safe manner getting all HSE practices and
procedures are adhered at all time.

In order to accomplish this, we will:

Ø Consult, listen and respond openly to our customers,
employees, neighbors, public interest group and those who
work with us.
Ø Work with others – our partners, suppliers, competitors and
regulators- to raise the standard of our industry.
Ø Openly report our performance, good or bad.
Ø Recognize those who contribute to improve HSE performance.

Our business plans include measurable HSE targets. We are all


committed to meeting them.
The HSE related Key performance indicators (KPI) are:
1. No record able cases
2. No days away from work (DAFW) cases or lost time accident
(LTA)
3. No environmental incidents

6.0 HSE ACTION PLAN


6.1 Safety Audit.
In obligation to achieve the HSE objectives and targets; it is
essential for the BCESB management team to joint formal
meetings, inspections and safety audits shall be held on
requirement basis.

All audit findings will be resolved immediately where possible. The


relevant OWNER Supervisors on any open action item shall notify
the Owner’s representatives and every effort shall be taken to
ensure the open action items are completed or rectified.

6.2 TOOL BOX MEETINGS


Tool Box Meeting are short informal meetings that shall be
attended by all workers; conducted by OWNER & BAKTICON
ENGINEERING SDN.BHD Supervisors assisted by the designation
OWNER Job Rep or the OWNER & BAKTICON ENGINEERING
SDN.BHD Safety personnel. Meeting will be conducted and
documented on regular basis.
BAKTICON ENGINEERING SDN.BHD will re-emphasis the
measurable HSE pledge / Targets to its employees and sub-
contractors.

6.3 HEALTH
The Health of all BAKTICON ENGINEERING SDN.BHD and
employees is an integral part of the site’s HSE policy. To
accomplish this end, the following shall apply:

6.3.1 SUBSTANCE ABUSE

OWNER reserves the discretionary right to require substance
abuse test to be conducted for any BAKTICON ENGINEERING
SDN.BHD employee involved in an incident or suspicious
behavior.

6.3.2 FIRST AIDERS


The site / owner to provide Emergency Response Team or
First Aiders in accordance with the “Factories and Machinery
Act 1967”.

6.3.3 PERSONNEL’S WORKING HOURS


All contract personnel’s working hours will be monitored to
ensure compliance with government statutory requirements.
Working hours will also be monitored to ensure personnel
continue to function safety if conditions are of a demanding
nature.

“Normal” working times will be from 0800-1700 hr


(48hrs/week) unless specifically dictated by the individual job
schedule / requirements.

6.3.4 INJURY / MEDICAL CASES


Injury cases / medical cases shall be referred to the OWNER
clinic immediately for appropriate medical attention.

6.4 SAFETY
Full consideration of the following must be observed:

6.4.1 INDUCTION AND TRAINING


All employees at all levels must undergo the safety induction
by OWNER before entry into OWNER Plant.

The safety induction shall include but is not limited to: -


• HSE Policy
• Personal Protective Equipment (PPE Procedure)
• Housekeeping
• Fire-prevention (Fire hazard)
• Site Access
• Incident / Accident reporting procedures (accident
response)
• Alcohol and drug abuse policy
• Hazardous Work Procedure
• Process Area ERP

• Permit To Work system (PTWS)

Training shall include the following:


All BAKTICON ENGINEERING SDN.BHD supervisors will
attend OWNERS’ facilitated Critical Safety Regulations
training covering the following safety regulations but not
limited to:
• Permit-to-work system
• Hot Work Permit
• Lockout & Tag out
• PPE
• Ready-for-work tag
• Confined space entry

6.4.2 CHEMICAL SAFETY DATA SHEET (CSDS)


BAKTICON ENGINEERING SDN.BHD and sub-contractors
will provide CSDS for any chemical to be used in OWNER
plant for evaluation or approval. Until this approval is
obtained, the chemical must remain outside the plant. Refer
to OWNER safety regulation on: CSDS Management
procedure for details of requirements (if available)

6.4.3 JOB SAFETY ANALYSIS & HIGH RISK


IDENTIFICATION.

• HIGH RISK IDENTIFICATION (HRI)


OWNER and BAKTICON ENGINEERING SDN.BHD will
identify all HRI jobs.

• JOB SAFETY ANALYSIS (JSA)


JSA will be:

Approved by : OWNER
All personnel involve should read and understand the
JSA prior to sign the sign off sheet. A sign off sheet will
be attached to the JSA and the example as per
attachment.

Examples of JSA jobs include, but are not limited to:


• Lifting operations with cranes
• Scaffolding erections and dismantling
• Hot work: welding and cutting
• Radiography

• Vessel entry/ confined space entry
• Work at height
• Excavation
• Chemical/ condensate splash potential

6.4.4 PERMIT TO WORK SYSTEM


BAKTICON ENGINEERING SDN.BHD will adhere to OWNER
permit to work system.

6.4.5 INCIDENT REPORTING AND INVESTIGATION


All incidents / accidents including near misses will be
immediately reported to the OWNER. A report (as per
attachment) will be submitted to OWNER within 24 hours
after the incident. BCESB will adhere to OWNER reporting
system and provide any further assistance requested by
OWNER.

6.4.6 EMERGENCY RESPONSE PLAN


In the event of an emergency, OWNER’S incident
Management Plan will take precedence. All BAKTICON
ENGINEERING SDN.BHD employees and contractors must
evacuate the premises and assemble at the nearest assembly
point which will be coordinated by OWNER’S Representatives.

BCESB supervisors are responsible to perform headcount


and report to Owner of any discrepancy.

6.4.7 PERSONAL PROTECTIVE EQUIPMENT (PPE)


All work site activities will necessitate the use of appropriate
and approved personal protective equipment. The type of PPE
required will be governed by OWNER’S Safety Regulation or
as stipulated on the permit-to-work and JSA.

Minimum requirement of PPE for BCESB personnel are safety


helmet, safety glass and safety shoe.

Issues of particular emphasis are:

• 100% tie-off (fall protection) is required for all scaffolding


erection and dismantling activities. Scaffolders shall use
lanyard for above activities.

• Tinted, dark or shaded safety glasses will not be worn on


site between 7.00 p.m. and 7.00 a.m. and / or in areas
where there is insufficient ambient light during other
hours.

• Wearing of full slicker suit with presence of operations


personnel during initial line breaking and the
requirements to be specified in the permit-to- work
including the need to barricade area below line breaking
activity if line break is at upper deck / level.

6.4.8 EQUIPMENT AND MACHINERIES.


All equipment and machineries e.g. cranes, excavators,
bulldozers, backhoes, forklifts, etc. will be inspected against a
checklist to ensure the equipment is in an acceptable
condition to operate. BCESB will keep the record of the
inspected equipment and machineries.

The following equipment, but not limited to will be inspected


by OWNER relevant personnel prior to approval for use:
• Cranes (with current certification)
• Slings, lifting tackles, chain block (with current
certification)
• Welding sets
• Portable electrical appliances
• Hand tools
• Grinders
• All compressors / generator sets (with current
certification)
• Forklifts
• Gas cylinders, hoses and protective caps
• Ladders
• Portable lights and electrical cables
• Fire extinguishers (with current certification)
• Portable electrical equipment:
An electrician on a regular basis will carry out inspection
of electrical cables and electrical tools. Generators and
welding sets will be earthed and fitted with an ELCB.
Industrial standard plugs and sockets will be used at the
job site

• Electrical power sources shall be turned off & unplugged
while electrical powered equipment is being repaired.
• Warning signs, “DO NOT OPERATE” / KEEP CLEAR” will
be placed on defective electrical equipment / electrical
panels.

6.4.9 FIRE PREVENTATION


Fire prevention shall be undertaken by application of the
following rules and procedures:
• Smoking is allowed at approved designated areas only.
• Never use unapproved heating, lighting or cooling
appliances
• Never allow combustible materials and debris to
accumulate
• Use suitable fire blankets when carrying out hot-work
where necessary.
• Ensure that all employees know the location of Fire
extinguishers and their use,
I.e. how to operate them, and also the procedures to
obtain assistance in the event of a fire.
• A dedicated trained Fire Watch with a Walkie-Talkie,
identified with a luminous vest and equipped with a water
hose line and fire extinguisher is required for all hot work
activities.

In the event of an uncontrolled fire at the job site, the fire


alarm shall be raised instructing all work at job site to be
stopped. Electrical equipment shall be turned off and all
personnel shall evacuate immediately to the designated
Emergency Assembly Points.

6.5 ENVIRONMENT
All activities at the site shall be carried out in an environmentally
acceptance manner as governed by the Environmental Impact
Assessment Report (1993) and Environmental Quality Act 1974
and regulations made there under.

Precautions and preventive measure to be taken would include the


following:

6.5.1 AIR QUALITY


All efforts shall be made to minimize and control dust any
other air pollution. OPEN BURNING, regardless size, is
strictly prohibited.

6.5.2 NOISE
Employing noise control methods wherever possible will
minimize noise impact.

6.5.3 DISPOSAL OF GARBAGE AND WASTES MATERIAL


BAKTICON ENGINEERING SDN.BHD will ensure that all
garbage and wastes are safely disposed. BCESB shall be
responsible for the removal of their own wastes, including
scheduled wastes in a safe manner:-
• Non scheduled wastes
• Scheduled wastes (Disposal by OWNER)
• Scrap metal

OWNER’s Goods Removal form will be used.

6.5.4 FUEL SPILLAGE


Ensuring careful transfer of fuel to equipment and motors
shall minimize potential pollution of split fuel. Drip trays
shall be provided under each transfer valve so that any spill
from the valve may be collected. If a spill occurs, badly
contaminated soil shall be collected and properly disposed of
as scheduled waste.

All storage of flammable fuels on site must be in a designated


location with appropriate warning signs, “No Smoking Signs”,
barricaded with restricted entry and appropriate spill
precautions must be taken, i.e. spill trays.

7.0 SECURITY
All personnel and vehicles will report to the OWNER security gates
before entry and when leaving the plant.

All materials entering and leaving the plant must be declared. A list of
all tools and equipment brought into the site must be forwarded to the
OWNER Security personnel. Completed Goods Removal Forms are
required for materials that are to be removed from the plant.

All vehicle entry is subject to inspection and a vehicle entry pass will
be issued. For vehicles used to ferry BAKTICON ENGINEERING
SDN.BHD from entrance to the temporary rest area, these vehicles
must be pre-inspected and details of the vehicles submitted to the
OWNER Security for preparation of a dedication temporary vehicle
entry pass.

8.0 HSE PERFORMANCE MONITORING


BAKTICON ENGINEERING SDN.BHD’s HSE Officer or designated
supervisor will be identified with a sticker on the individual’s safety
helmet with the word “SAFETY OFFICER”. This person will ensure that
BAKTICON ENGINEERING SDN.BHD furnishes the following report
daily and totaled at the end of the month:-
• Man- hours worked.
• Incident / accident reports (if any)
• Environmental incidents (if any)
• Follow –up action / closure reports to any BAKTICON
ENGINEERING SDN.BHD violation reports, safety audit action
items and non-conformances to OWNER safety rules &
regulations.

SAFETY STATICS
BAKTICON ENGINEERING SDN.BHD shall send a Statistics Report to
OWNER at the end of the project.

9.0 ENFORCEMENT ON BREACH OF HEALTH AND SAFETY RULES


BAKTICON ENGINEERING SDN.BHD will take a serious view on any
employee who fails to comply with BAKTICON ENGINEERING
SDN.BHD’s or OWNER’s Safety Rules, Regulations and Safe Working
Procedures as outlined in the Safety Orientation, JSA, permit-to-work,
toolbox meetings, on – the- job instruction, etc.

For any non-compliance of safety procedures, BAKTICON


ENGINEERING SDN.BHD will immediately initiate the following
disciplinary actions: -

1st time - stop personnel’s work activities until rectified

2nd time - written warning with suspension if necessary from


work for 2 days for employees concerned. A safety
re-orientation shall be given.

3rd time - if the non-compliance is repeated, the involved
employee will be removed from the site permanently.

Unsafe condition - “Stop Work” will be ordered. Immediate


rectification must be completed. The area will be re-
inspected and approved by the inspecting authority
or OWNER Safety
