August 1, 2011 Dr. Diosdado M. San Antonio Superintendent City Division Schools Naga City Sir: Greetings! We, the researchers from Bachelor of Science in Nursing 4A from Universidad de Sta. Isabel, are conducting a research on Factors Affecting the Growth and Development of Preschool Children in a Private and Public Schools of Naga City. The chosen schools are Universidad de Sta. Isabel and Tinago Central School. Part of this research is to get the preschool childrens Body Mass Index (BMI) and to administer the Metro Manila Developmental Screening Test (MMDST). Another part of the research is to conduct a survey through a questionnaire. In connection with this, we would like to ask for your approval of the said questionnaire to be answered by the advisers of the preschool children and for the researchers to visit the school and to mingle with the students during class hours for the gathering of the preschool childrens BMI and MMDST results.
