OBJECTIVES General Objective: The main goal of the group is to be able to present an extensive and comprehensive case study

of our chosen client that would present a comprehensive discussion of Schizophrenia Undifferentiated to yield important information for the case study. Specific Objectives: 1. Identify the common factors that may contribute or precipitate in the occurrence of Schizophrenia Undifferentiated. 2. Evaluate the presence or absence of signs and symptoms seen in the patient in relation to the mental disorder. 3. Gather pertinent data about the client through detailed chart taking, and effective therapeutic communication and interaction with the client and his significant other. 4. Present the psychodynamics of the clients diagnosis by recognizing its predisposingand p r e c i p i t a t i n g f a c t o r s w i t h a p p r o p r i a t e r a t i o n a l e s ; t o t r a c k d o w n t h e s i g n i f i c a n t e v e n t s during the clients developmental stage as shown in the psychodynamics. 5. Formulate effective, specific, measurable, attainable, realistic and timebounded nursingcare plans base on identified actual and potential nursing problems. 6. T r a c e t h e h e a l t h h i s t o r y o f t h e c l i e n t a n d f a m i l y i l l n e s s e s ( p a s t a n d p r e s e n t ) t h r o u g h a genogram. 7. Present the medications given to the client, including their respective modes of action, indications, contraindications, side effects, adverse reactions, nursing responsibilities, and importance to the clients condition. 8. P r e s e n t t h e a n a m n e s i s b y t h o r o u g h g a t h e r i n g o f t h e c l i e n t s p e r t i n e n t p e r s o n a l d a t a , appropriate selection of informants, and familial history tracing. 9. Present both ideal and actual management in regards to medical and nursing management in relation to client. 10. Determine the clients possible actual and ideal prognosis.

IFFERENTIAL DIAGNOSISSCHIZOPHRENIASchizophrenia is one of a cluster of related psychotic brain disorders. It is a combinationof disordered thinking, perceptual disturbances, behavioral abnormalities, affective disruptions andimpaired social competency. The diagnosis of a particular subtype of schizophrenia is based on theclinical picture that occasioned the most recent evaluation or admission to clinical care and maytherefore change over time. They are defined by their symptomatology. The disorder lasts for atleast 6 months and includes at least one month of the active phase symptoms namely two or more of the following: hallucinations, disorganized speech, catatonic behavior, negative symptoms). Thesubtypes are:295.30 Paranoid Type295.10 Disorganized Type295.20 Catatonic Type295.90 Undifferentiated Type295.60 Residual Type
