Shirk or associating others in worship with Allah.

Verily, Allah forgives not (the sin of) setting up partners (in worship) with Him, but He forgives whom He wills, sins other than that, and whoever sets up partners in worship with Allah, has indeed strayed far away [An-Nisa 4:116]

That includes praying to the dead, seeking their help, making vows and offering sacrifices to them or to the jinn or to the grave.

Sets up intermediaries between himself and Allah, asks them to intercede, and puts his trust in them

Unquestionably, for Allah is the pure religion. And those who take protectors besides Him [say], "We only worship them that they may bring us nearer to Allah in position." Indeed, Allah will judge between them concerning that over which they differ. Indeed, Allah does not guide he who is a liar and [confirmed] disbeliever. [Az Zumar 39:3]

Does not regard the mushrikeen as kaafirs, or doubts that they are kaafirs, or regards their way as correct

Indeed, the religion in the sight of Allah is Islam. And those who were given the Scripture did not differ except after knowledge had come to them - out of jealous animosity between themselves. And whoever disbelieves in the verses of Allah , then indeed, Allah is swift in [taking] account. [Ali Imran 3:19]

Believes that anything other than the teaching of the Prophet is more complete than his teachings, or that the rulings of anyone else are better than his rulings such as those who prefer the rule of false laws to his rulings

Then is it the judgement of [the time of] ignorance they desire? But who is better than Allah in judgement for a people who are certain [in faith]. [Ali Imran 3:19]

Indeed, We sent down the Torah, in which was guidance and light. The prophets who submitted [to Allah ] judged by it for the Jews, as did the rabbis and scholars by that with which they were entrusted of the Scripture of Allah , and they were witnesses thereto. So do not fear the people but fear Me, and do not exchange My verses for a small price. And whoever does not judge by what Allah has revealed - then it is those who are the disbelievers. *Al Maidah 5:44]

[As for] the thief, the male and the female, amputate their hands in recompense for what they committed as a deterrent [punishment] from Allah . And Allah is Exalted in Might and Wise. [Al Maidah 5:38]

Section 380 of the Penal Code Prison up to 10 years and fine

Section 380 Penal Code (Revised 1997) Act 574, read together with Section 34 of thereof, and sentenced as follows: a. Japar bin Bakong - 24 months imprisonment effective from 13 October 2006 and a fine of RM5,000 in default 3 months imprisonment. http://www.kehakiman.gov.my

The [unmarried] woman or [unmarried] man found guilty of sexual intercourse - lash each one of them with a hundred lashes, and do not be taken by pity for them in the religion of Allah , if you should believe in Allah and the Last Day. And let a group of the believers witness their punishment. [ An Nuur 24:2 ]

Hate any part of that which the Prophet brought, even if he acts in accordance with it

That is because they hate that which Allah has sent down (this Quraan and Islamic laws); so He has made their deeds fruitless [Muhammad 47:9]

Make fun of anything in the religion of the Prophet or makes fun of any texts that refer to rewards or punishments

And if you ask them, they will surely say, "We were only conversing and playing." Say, "Is it Allah and His verses and His Messenger that you were mocking?" Make no excuse; you have disbelieved after your belief. If We pardon one faction of you - We will punish another faction because they were criminals. [At-Tawbah 9:65-66]

Sahr (witchcraft) including spells to turn one person against another or to make someone love another. Whoever does this or approves of it is a kaafir.

And they followed [instead] what the devils had recited during the reign of Solomon. It was not Solomon who disbelieved, but the devils disbelieved, teaching people magic and that which was revealed to the two angels at Babylon, Harut and Marut. But the two angels do not teach anyone unless they say, "We are a trial, so do not disbelieve [by practicing magic]." And [yet] they learn from them that by which they cause separation between a man and his wife. But they do not harm anyone through it except by permission of Allah . And the people learn what harms them and does not benefit them. But the Children of Israel certainly knew that whoever purchased the magic would not have in the Hereafter any share. And wretched is that for which they sold themselves, if they only knew. [Al-Baqarah 2:102]

Supporting the mushrikeen and helping them against the Muslims.

O you who believe! Take not the Jews and the Christians as allies (protectors, helpers), they are but allies of each other. And if any amongst you takes them (as allies), then surely, he is one of them. Verily, Allah guides not those people who are the Zaalimoon (polytheists and wrongdoers and unjust) [Al-Maa'idah 5:51]

Whoever believes that some people are allowed to operate outside the law of Muhammad, is a kaafir.

And whoever seeks a religion other than Islam, it will never be accepted of him, and in the Hereafter he will be one of the losers. [Ali-Imraan 3:85]

Turning away from the religion of Allah, not learning it and not acting in accordance with it.

And who does more wrong than he who is reminded of the Ayaat (proofs, evidences, verses, lessons, signs, revelations, etc.) of his Lord, then turns aside therefrom? Verily, We shall exact retribution from the Mujrimoon (criminals, disbelievers, polytheists, sinners). [As-Sajdah 32:22]

No guidance in life

That is because they believed, and then they disbelieved; so their hearts were sealed over, and they do not understand. [ Al-Munafiqun 63:3 ]

All good deeds are rejected

The example of those who disbelieve in their Lord is [that] their deeds are like ashes which the wind blows forcefully on a stormy day; they are unable [to keep] from what they earned a [single] thing. That is what is extreme error. [ Ibrahim 14:18 ]

Forever remain in hell

And whoever disobeys Allah and His Messenger and transgresses His limits - He will put him into the Fire to abide eternally therein, and he will have a humiliating punishment. [ An Nisaa 4:14 ]

Unforgiven

Indeed, those who disbelieve and commit wrong [or injustice] never will Allah forgive them, nor will He guide them to a path. [ An Nisaa 4:168 ]

Forbidden from entering heaven

And the companions of the Fire will call to the companions of Paradise, "Pour upon us some water or from whatever Allah has provided you." They will say, "Indeed, Allah has forbidden them both to the disbelievers. [Al-'A`rf 7:50 ]
