At least 75 minutes of strenuous exercise each week for two months

At least 90 minutes of moderate exercise each week for two months

Continue to Eat a Healthy Diet (Also helps lower cholesterol)

Emphasizes fruits (2 cups per day), vegetables (2.5 cups per day), and whole grains (1.5 cups
per day)

Low-fat dairy products and low-fat proteins, such as poultry, fish and legumes

Moderate amounts of healthy fats, such as unsalted nuts, and vegetable and olive oils

4.

Limit saturated fats (under150 calories)

Follow this diet for two months

This plan was instituted in agreement with Ramon on November 28 th, 2016. If the plan is
followed, Ramon should lose about 10 pounds.

Recommendation for Medical Procedures

Right now, there is no recommendation for a medical procedure

Re-evaluation will be conducted at the next appointment on


January 30th, 2017 to determine if a procedure will be needed in
the future

Recommendation for Medications

Take 10 mg of Atorvastatin (Lipitor) once daily for two months

Atorvastatin can be taken with or without food

Re-evaluation of the effectiveness of the medication will be


conducted at the next appointment on January 30 th, 2017

Testing and Goals
Initial appointment for Ramon was on November 28 th, 2016
Things to check for at the appointment on January 30 th, 2017 (2 months after
initial appointment):

Make sure blood pressure levels have not changed (Should be 120/71 mmHg)

Make sure cholesterol levels have not significantly increased or decreased

Total cholesterol should be around 190 mg/dL

LDL cholesterol should be around 90 mg/dL

HDL cholesterol should be around 45 mg/dL

Make sure Ramon is completing the amount of exercise recommended by


performing a stress test on his heart

Are there any significant findings from the stress test that could hint at heart
disease?

Did Ramon reach the goal of losing 10 pounds within the two month time
period?
