Observation
A worker is found working at height changing a damaged roof by using a
Mobile Elevated Work Platform in the ware house but the operator has
placed the vehicle on an uneven ground surface which leads to improper
distribution of load on the out riggers and can lead to possible over turn of
vehicle (or) can create a toppling effect on cradle resulting in collapse of the
vehicle causing severe injury or fatality of operator and even damage to
surrounding workers , property and vehicle which creates a loss to the
organization and is breaching Indian Standards IS 14734-1999 which can
lead to possible imposition of fines and penalties to the organization against
the breach of law and the organization.
Recommendation
Provide base plates under the outriggers to make it even and provide
refreshed training to operators, advice supervisor to carry out such activities
under supervision and also provide barricade to prevent contact of vehicle
with moving workers and provide appropriate signage’s to make workers
aware which would cost you around INR 2500.which reduces the
consequence and likelihood of hazard
