What Happened?
On 4th May 2012 a Heavy Equipment Mechanic was
assigned to change the Left side front tire of a Bus.
While removing the nuts on the rim of the tire with a
Wrench ,he found that the nuts are jammed . So he
used a hammer as support . While applying force on
the wrench handle using both the hands ,the nut got
lose and his thumb got caught- between the hammer
handle and the wrench handle resulting in a crush
injury on his left hand thumb finger.

Key Findings of the Incident


 No Safe Work Procedure developed for
Demounting and Mounting truck and bus
tires.
 Insufficient Supervision / Safety Awareness
among the employees involved in the job .
 Under estimating the risk involved in the job.

Key Learning Facts


 Tire and wheel servicing can be dangerous and
must be done only by trained personnel using
proper procedures and tools.
 Supervisor / Foreman shall conduct Daily Task
Plan and explain the hazards and the control
measures available before starting any
activities.
 Effective Supervision to be provided while
performing any such activities.
 Conduct Safety Awareness sessions during
Tool Box Meetings ,and ensure employee are
adequately trained to recognize the Risks.
