ADOPTED COMMUNITY
Barangay Kayquit III, being one of the largest
and most populated barangays in the municipality
of Indang is divided into seven (7) Puroks, each one
headed by a Barangay Kagawad under the
supervision of the Punong Barangay.

INTRODUCTION
• One thing that has a significant role in our life is
health. It has something to do with the physical,
medical, relationship, mental, and emotional aspect of
an individual. The most important thing to do to boost a
better quality of living is to attain the reservation of
having a healthy lifestyle.
• The awareness towards the significance of social
determinant of health is being observed by the public
health community in which both the community and
public health help in reducing the health risk.
OBJECTIVES
Specifically, this case study aims to:
• Accumulate information regarding the demographic profiles of the citizens.
• Determine the common health problems that affects the community in
percentage.
• Assess the factors that induce certain health problems that are evident among
the community.
• A concrete plan to address the determined health problems.
• Manage the factors affecting the health condition of the community through
conducting appropriate programs.
• Evaluate the effectivity of implemented programs according to their impact
towards the state of health of the community.

• 85% of the Households do not practice Annual Laboratory Testing
• Population is prone to Non-Communicable Diseases with percentages of
68% in the past and 84% in the present.
• 81% of the couples do not practice any Method of Contraception
• Neighborhood is 70% Congested
• 63% of the households are not aware of their Blood Type
• 54% of the Population are Overweight (25%), Obese (10%) and
Underweight (19%).
• 43%drinking water supply is Tap
• 42% of the population had never went for Oral Check Up.
• 32% of refuse and garbage containers are open

BLOOD SUGAR LEVEL
• An alarming rate of 85% of the population
do not conduct annual laboratory testing,
and 35% of the populations have a body
mass index which falls above normal. It
was concluded that blood sugar levels
must be tested in order to find out if the
residents’ sugar levels were still average
and to prevent the early stages of diabetes.

SUMMARY, RESULTS, CONCLUSION
AND RECOMMENDATION
The first program conducted was
entitled “One, Two, Three, SMILE!: Dental
Health Seminar and Consultation”, wherein
Doctors who specialized in dental health
such as, Dr. Jean Salazar, Dr. Floracel
Dagdag and Dr. Corazon Bacerra were
invited. A total of 52 residents joined the
program where children have the highest
percentage followed by elders and only few
young adults.
The second program was entitled “Ipakonsulta
ang Mata upang ang Paningin ay Lalong
Gumanda: A Seminar About Cataract
Awareness with Professional Consultation“. Mr.
Anthony Christian S. Sabino, RN MBA, Dr.
Hermie O. A lot and Mr. Kenny John C.
Fortunawereinvited. One hundred (100)
residents were accommodated and among
the 100 residents those that are ages 40 and
above were given free reading glasses.
The third program was entitled “Random Blood
Sugar Testing Program: Diabetes ay Sugpuin,
Pangangatawan ay Palakasin, Healthy Lifestyle
Iyong Ugaliin.” This program was supervised by
Mr. John Jeron B. Datoy, RMT and Ms.
Stephanee M. Gramonte, RMT. Fifty (50)
residents of Purok 3 and 4, Barangay Kayquit III,
Indang, Cavite joined the program but only 47
residents successfully had their blood glucose
tested.
Lastly, a disengagement program was held to
show gratitude and appreciation to the
Barangay Officials who allowed, helped and
supported the programs held in the community
of Purok 3 and 4 of Kayquit 3, Indang, Cavite.
